Job Description
Refugee Care Connector will assist refugees and other humanitarian immigrants living in Prince Georges and Montgomery Counties (Maryland) with connections to medical care, ongoing health services, and associated administrative activities (e.g. registration/enrollment paperwork) for issues related to Maryland Medical Assistance (MA) such as Managed Care Organization (MCO) and primary care physician selections, and for the navigation of the healthcare system and local social services.
Essential Functions
Develop a healthcare referral system, comprised of providers who are culturally and linguistically aware, as well as, geographically located in proximity to where humanitarian immigrants are living.
Responsible for coordinating and troubleshooting issues related to transportation, language access, emergency medical issues, and scheduling of appointments. The incumbent will also
Track all clients' MA enrollment and activation dates and follow-up on cases where MA activation is not timely.
Facilitate the continuity of healthcare for humanitarian immigrants for their first six (6) months.
Work closely with area resettlement agencies, health screening clinics, and MA enrollment sites.
Other duties as assigned.
Qualifications
Qualifications and Education Requirements Bachelor's Degree (four year college or technical school) Required, Field of Study: Social Work, Public Health, Human Services or related field
Master's Degree Preferred, Field of Study: Social Work, Public Health, Human Services or related field
Experience in partnership development with health service organizations.
High level of energy, self-confidence, and commitment to continuously improve service delivery.
Proven ability to work independently and as a team member in a fast-paced, cross-cultural environment.
Possess reliable transportation, valid US driver's license, and active vehicle insurance.
Preferred Skills Two (2) plus years of experience in Social Work, Human Services or related field
Experience in domestic refugee resettlement programs or closely related social service delivery programs highly desirable.
Experience working with immigrant populations with limited English proficiency highly desirable.
Knowledge of Maryland Medical Assistance/Health Choice is a plus.
Computer Skills: Microsoft Applications (Access, Excel, Word, and Power Point);
Knowledge of databases
Other Requirements: Fluency in one or more of the following languages- Dari, Pashto, French, Spanish, Swahili
Excellent written and verbal skills
Self-starter with the ability to work independently and as a member of the team
