Description

Practice Integration / Customer Success Manager

Summary:

We are seeking a highly motivated and experienced Manager to lead the integration of practice acquisitions into Ascend Medical. In this highly visible and critical role, you will oversee the planning, development, and implementation of the integration process between Ascend's innovative practice model and newly acquired physician practices. Working closely with Executive leaders (CEO, President & Head of Sales) you will be responsible for supporting practice owners and their staff through the integration process:

• Ensuring alignment with standard workflows;

• Streamlining referrals & clinical integrations;

• Transitioning technology platforms (incl. EMR migration) and facilitating efficient data exchange;

• Enhancing patient care delivery and operational efficiency.

This role will challenge you to excel in project/process management and communication, connecting the right experts at the right time to optimize the success of our integration process, as well as excel in problem solving - predicting and overcoming barriers.

Company Summary :

Ascend Medical, founded in 2021, has a mission to elevate body, mind and soul. Our advanced primary care clinically integrates behavioral, metabolic and lifestyle aspects of health to keep you well physically, mentally and emotionally. Ascend started as a convenient, hybrid delivery model spanning digital and at home modalities, meeting patients where they are: at home, at work or anywhere else. We have since evolved and now partner with traditional primary care providers to augment their offering, allowing patients to experience Ascend through brick-and-mortar, digital and at-home channels.

Unlike your typical healthcare system, Ascend is designed to innovate. Innovation starts with a culture that is excited to reimagine healthcare and is not afraid to disrupt. We try to avoid any micro-management of employees but rather embrace a core set of values that empower our team-members to make smart decisions:

Our values:

• Patients before Profits

• Empathy before Expedience

• Progress before Stagnation

• Team before Individual

• Safety before Everything

At Ascend Medical, you'll be part of a team of caring and innovative professionals who are forging a new path in healthcare - one that is patient-centered, affordable, and highly convenient. Ascend boasts the highest patient satisfaction scores in the industry (net promoter score consistently above 95) and we excel in delighting patients.

Essential Duties and Responsibilities:

• Establish and maintain a comprehensive integration strategy, playbook & roadmap that is aligned with the company's overall business objectives.

• Oversee the integration roadmap, communication plans, and the organizational design and implementation for integrating newly acquired practices: clinically (including ancillary care, clinical protocols, referral process, care coordination), administratively (communications, billing, coding, scheduling) and technologically (EMR integration, phone systems, marketing technology).

• Oversee the integration lifecycle, including needs assessment, workflow development, execution, and ongoing maintenance and refinement.

• Engage with the Business Development team pre-close to identify early integration opportunities, understand practice pain points and key personnel, and tailor the integration process to address specific needs of new practices.

• Monitor performance of newly integrated practices, new clinicians, and new services and troubleshoot any integration issues to ensure seamless operations.

• Participate in the due diligence process for potential partner practices, evaluating key issues, assessing integration challenges, crafting detailed post-closure plans, and highlighting best practices or potential disconnects that could destroy value or create risk.

• Be the primary point-of-contact for physician owners (and their teams) going through the integration process, providing an excellent level of customer service throughout an often challenging and emotional experience.

• Foster strong relationships with internal stakeholders across all departments (e.g., finance, IT, operations, clinical) and manage the pace of integration, ensuring timely decision-making, and providing accountability for deliverables.

• Coordinate with VP of HR to ensure successful training and onboarding of newly acquired staff.

Location :

• Atlanta Metroplex area

• This position is mostly remote but will require some travel with sporadic onsite visits to practices being acquired across Georgia in particular (up to 25%)

• Relocation will be considered for the right candidate

Company benefits:

We?offer a competitive benefits package to our employees:

• Medical

• Vision

• 401k w/ Match

• Free healthcare with Ascend including telemedicine options

• Generous PTO Package
