The posted compensation range of $47.09 - $64.85/Hour is a reasonable estimate that extends from the lowest to the highest pay Pathways Home Health & Hospice in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. Pathways Home Health & Hospice may ultimately pay more or less than the posted range as permitted by law.
POSITION SUMMARY:
• Maintains a minimal patient caseload.
• Facilitates interdisciplinary communication and teamwork to identify and manage cases involving risk and/or complex psychosocial issues.
• Assures that the Social Services program meets quality, regulatory and budgetary goals.
• Provides personnel management for assigned social services staff, including hiring, orientation, evaluation, staff productivity monitoring, staffing levels and payroll authorization.
• Assures qualified preceptors are identified to mentor and train new staff.
• Provides or makes provision for clinical and organizational supervision to those providing medical social services, including MSW interns assigned to the social services program.
• Assures appropriate productivity levels for assigned staff within budgetary guidelines.
• Assures accurate and appropriate documentation that meets quality, regulatory, and reimbursement requirements.
• Coordinates with the Manager of Clinical Support Services to ensure Social Services collaborates effectively to provide quality patient care.
• Coordinates with the Director of Quality Improvement and Education to assure policiesand procedures are communicated and that staff are educated and informed, as required.
• Coordinates with education staff to assure provision of in-service programs for social services and spiritual care staff. Provides adequate opportunity for education and ensures that assigned staff meets in-service requirements.
• Coordinates with the Coordinator of Volunteer Services and/or Volunteer Department Managers to assure the effective utilization of volunteers.
• Interfaces with Human Resources, Administrative Services, and IT and Telecommunication Coordinators to resolve safety, facilities, supply, equipment and IS/telecommunications issues.
• Develops and maintains effective working relationships with social service agencies and other community providers.
• Manages eligibility of hospice, home health, and private duty services for patients who have inadequate insurance coverage and require financial assistance.
• Understands and supports Pathways Home Health and Hospice philosophy and principles and implements these in professional practices.
• Participates in quality improvement activities related to areas of practice as appropriate.
• Maintains current knowledge of regulations pertinent to the delivery of home health and hospice services and conducts department operations in a manner to be in compliance with such regulations.
• Performs other duties as assigned consistent with skills and training and the mission and goals of Pathways.
QUALIFICATIONS:
• Licensed Clinical Social Worker in the State of California.
• Master of Social Work degree from an accredited university.
• Minimum of three years within the last five years of field experience in Homecare/Hospice or related community-based health care setting.
• Minimum of one year experience in the provision of social work services to terminally ill patient’s preferred.
• Minimum of two years of supervisory/management experience.
• Current California driver’s license and automobile insurance.
