Primary Duties and Responsibilities Actively (physical presence and verbal expressions) participates in multidisciplinary quality and service improvement teams focused on specific problems associated with resident care, goals and approaches. Maintains cultural competency, and assists other staff members to do the same. Serves as a leader in the area of cultural competence for the facility. Seeks supervision and/or consultation on how to handle problems that may develop in the area of an absence of cultural sensitivity of the part of other staff members.

Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards and safety standards. Complies with governmental and accreditation regulations.

Develops and maintains a community resource file and provides information and referrals as needed. Resource file should include agencies that offer voluntary services; Social Services staff should utilize the resources in their file frequently to supplement their Unit's services, and update the file regularly to verify contact names, telephone numbers and email addresses.

Develops relationship with the family/associates, discusses mutual expectations, and incorporates the family/associates in the care planning. Explains to the family the importance of fostering independence and supporting the resident's care plan, checks with the family/associates on their understanding of what this means, and what it means to them. Develops and maintains therapeutic group activities such as family support group. Assists the family through the grieving stage including acceptance of the post-admission process. Develops cultural competence with each resident and their family/associates, noting culturally bound elder care and grieving norms.

Develops social service component of the care plan and identifies specific problems, goals, and approaches. Documents same in the social work portion of the care plan within 24 hours of the care plan meeting, and initiates and updates MDS. Updates the care plan as needed; brings to the attention of pertinent staff when updates are required and/or made.

Helps residents cope with the social and psychological aspects of their impairments and requests clinical social worker of psychologist to attend to the resident when clinical issues are assessed; documents such requests in resident's chart immediately, and brings request to Supervisor's attention.

Maintains up-to-date required records, reports, statistics, and files. Plans ahead to meet all deadlines; requests deadline info from Supervisor as necessary. Tracking complaints, grievances within a specified time frame associated with each issue. Follow-up with each issue within a specified timeframe.

Meets with residents and families/associates to complete a full social history and develop an initial assessment identifying medically related social and psychosocial needs. Demonstrates cultural competence with residents, families/associates and other staff.

Participates in interdisciplinary care conference at time of admission and at prescribed intervals to review, develop and, when appropriate, revise the overall plan of care. Make clear verbal expressions of assessments and updates on resident's specific problems, care and approaches/interventions. Social Service note must be in the residents chart within 48 hours of admission.

Participates in meetings and serves on committees to represent the department and nursing center in community outreach efforts. Attend conferences, seminars, and workshops seeking further training in gerontology/geriatrics, ethics, cultural sensitivity, social welfare and Medicare/Medicaid policies and computer technology. Provide information (brochures, newsletters) about Good Samaritan Nursing Center at any and all outreach events.

Performs other duties as assigned; other duties would include identifying and requesting the agency to purchase current literature, texts and professional journals that help to keep the Social Services Unit staff abreast of developments in social work with geriatric patients, care-giving issues, funding sources and social welfare polices.

Provides individuals with support as needed; evaluates the need for possible psychiatric intervention and works with geropsychiatric consultant to make appropriate referrals. Document in resident's chart such assessments, requests for psychiatric intervention, and resulting referrals; informs Supervisor of such interventions.
