Our services encompass 14 supportive housing sites, partnerships with rapid and long-term rehousing providers, and first-class behavioral health services.

As a Behavioral Health Clinician, you will be embedded on-site, offering a wide range of behavioral health interventions to adults who are navigating the challenges of transitioning to permanent housing, dealing with mental health issues, striving for healthy aging in place, and addressing substance abuse needs.

Responsibilities:

In this role, you will:

Maintain a regular caseload and deliver professional, culturally relevant support, advocacy, information, and referrals to program participants.

Complete initial and ongoing assessments, diagnoses, and treatment planning in collaboration with your assigned caseload.

Regularly meet with each client and provide Medi-Cal billable mental health services, including assessment, rehabilitation skills building, diagnosis, referral, consultation, brokerage, counseling, and crisis management.

Collaborate with Housing Support Services to serve clients who may need support with supportive housing intake, lease signing procedures, property management lease enforcement, and outreach and engagement strategies for high-needs clients.

Ensure accurate and up-to-date documentation of care plans, client interactions, progress toward goals, and exit plans using the Avatar electronic charting and billing system.

Requirements:

A master's degree in social work or a related field is required, along with Board of Behavioral Sciences registration/licensure (AMFT, APCC, ACSW, LMFT, LPCC, or LCSW).

Prior experience in providing services to homeless populations, as well as other very low-income families and individuals.

A strong commitment to and knowledge of harm reduction practices.

Excellent written, verbal, and interpersonal communication skills.
