Position Overview

The Program Coordinator of Community Supports coordinates the following Office of People with Developmental Disabilities (OPWDD) Home and Community-Based Services (HCBS) Waiver programs: Community Habilitation, Respite and Housing Rental Subsidy (HRS). This includes but is not limited to overseeing a caseload of people supported, directly supervising Direct Support Professionals (DSPs), promoting the well-being and independence of people supported, and ensuring compliance with regulations and agency policies. Performance Management: Provide meaningful and documented supervision to Direct Support Professionals (DSPs) including ensuring evaluation completion and support as needed. Review and approve employee timecards and review payroll reports.

Case Management: Oversee a caseload of people supported in Community Habilitation, Respite and HRS. This includes ensuring the health and safety of those supported and overall program satisfaction.

Service Planning and Coordination: Schedule and facilitate person centered meetings to collaboratively develop Staff Action Plans for those supported in Community Habilitation. Coordinate and/or facilitate support meetings (i.e., Interdisciplinary Team Meetings, Staff Introduction Meetings), as needed. Attend all necessary meetings (i.e. LifePlan Meetings), as needed. Record meeting minutes.

Relationship Development: Develop meaningful relationships with people supported, advocates, and family members by being in contact at minimum on a weekly basis. For those enrolled in HRS, schedule and facilitate at least 1 in person apartment check for each person on caseload per month. Complete spot checks for all others supported on caseload as needed, at minimum 2x a year per person. Assist with completion of satisfaction surveys.

Compliance: Ensure compliance with relevant OPWDD regulations and agency policies. This includes but is not limited to document outreach/collection/upload (i.e., Life Plans, Level of Cares, Level of Supervision, etc.), completion of DDP2s, service delivery, daily notes, and HRS required documentation (OPWDD QA Checklist and OPWDD Participation Agreement).

Engagement: As needed, provide direct service to people supported in need, specifically for those we support who are in need of assistance and are awaiting a permanent staff to be assigned. Assist with DSP recruitment, interviews, and new hire process.

Monitoring and Follow Up : Assist people as needed with resolving issues with landlords and other tenants/roommates that could jeopardized housing sustainability. Remain in constant communication with support team to provide updates as needed. Consistently check in on progress towards achieving goals.

Emergency Response : Assist with implementing systems for responding to crises and/or emergencies for people supported, especially for those who live independently.

Advocacy: Advocate for the needs and rights of people supported, ensuring they are receiving adequate services and support. Assist people supported with identifying additional resources and/or services.

Reporting: Ensure proper reporting, investigation, and review of all incidents relative to OPWDD Part 624 regulations and the New York State Justice Center.

Professional Development: Complete all mandatory and assigned trainings as needed. Take initiative in asking for support in areas needed for development and enroll in training opportunities to assist with accomplishing professional goals.

All other duties deemed necessary by the supervisor.

Job Skills

Qualifications Age Requirement : Minimum age of 18.

Experience:

1+ years working in OPWDD HCBS Waiver programs including but not limited to Community Habilitation, Respite and HRS, required.

1+ years in a supervisory/leadership role, preferred.

Educational Qualification: High School Diploma, required. Associate degree in related field (i.e., Social Work, Mental Health, etc.), preferred.

On Call: Flexible availability

Drivers Status: Valid Driver's License & personal vehicle required

Physical Demands: The nature of tasks may vary based on the individuals being supported. This may include assisting with toileting and hygiene, which could involve lifting. Reasonable accommodations will be considered to enable individuals with disabilities to fulfill essential job functions.
