**Case Manager purpose:**
The Case Manager is responsible for outreach, engagement, and providing support and linkage to wellness/recovery resources, both in person and virtually to assist clients in reaching the optimal level of wellness and functional capability. The Case manager works with highest risk Medi-Cal members with the most complex medical and social needs, connecting with all their providers to coordinate areas of care and support.
**Required Qualifications:**
+ Bachelor's degree in psychology or a related field, or equivalent training and experience that would provide the ability to perform the duties described above.
+ One year or more of case management or related experience.
+ Fluent in English and one of the Santa Clara Country threshold languages (Spanish, Tagalog, Vietnamese, Mandarin, Cantonese) preferred.
+ Trauma informed training preferred.
**Preferred Qualifications:**
+ Familiarity with community-based resources.
+ Excellent problem-solving and organizational skills.
+ Understanding of cultural issues, community resources and family dynamics.
+ Ability to communicate well with people of diverse cultural, professional, and experiential background.
+ Ability to establish and maintain effective work relationships as part of a multidisciplinary team.

**Pay Details:** $25.00 to $27.00 per hour
