The Clinical Case Manager works with a team to provide direct outreach, linkage, and retention services to homeless individuals, aiming to promote recovery and community integration. Responsibilities include:
Conducting comprehensive biopsychosocial assessments and developing intervention plans.
Collaborating with clients to create individualized treatment plans.
Offering ongoing case management to monitor progress and adjust treatment plans as needed.
Providing field-based supportive services addressing mental health, functional, and substance use barriers for housing stability.
Consulting with LCSW and supervisors for crisis and suicide risk assessments and interventions.
Responding to the 24/7 After Hours Response phone line.
Preferred Qualifications:
Current licensure or registration with the California Board of Behavioral Sciences (e.g., ACSW, LCSW).
Minimum Qualifications:
Master’s Degree in Social Work or equivalent behavioral health degree eligible for licensure through the CA Board of Behavioral Sciences or approved by program funder (e.g., MSW, MFT).
One year of experience in healthcare, case management, homeless services, or with vulnerable populations.
Mandatory Requirements:
Employment eligibility verification.
CPR/First Aid training (current or obtainable).
Completion of Tuberculosis Test, Background Screening, and Drug Test.
Driving is essential; candidates must have reliable transportation, a valid driver’s license, and proof of insurance for personal vehicles used for work.
