About the Role
Vanna Health is seeking a Care Coordinator who can bring their passion for supporting individuals living with serious mental illness throughout recovery. This role will be focused on coordinating various parts of the recovery journey for our members, including discharge planning, care coordination, and evaluating member eligibility for and enrolling them in various benefit programs.
We are looking for a Care Coordinator who has worked with individuals living with serious mental illness and understands the unique needs of this community. In this role, the Care Coordinator will be working with the multi-disciplinary community team to plan and coordinate services for members. Alongside Team Leads, Coaches, Clinicians, and other specialists, the Care Coordinator will have the resources and support they need to help empower community members to thrive.
Vanna Care Coordinators will help develop an innovative, recovery based program, and we deeply value the insight that you will bring to the team. Vanna will leverage our purpose-built, mobile-first technology to redefine what it means to work in community health.
Responsibilities
Participate in Vanna outreach efforts at parter CRCs, hospitals, and homeless outreach
Lead discharge planning for Vanna community members at ERs, CRCs, and hospitals
Coordinate referrals and support appointment scheduling with local primary care, behavioral health, and other provide types for members when required
Provide appointment reminders to members and ensure that they have the means to attend their appointments, such as transportation
Assess member needs and support systems, including social drivers of health
Learn about member enrollment in government and other support programs (SSDI, welfare, housing)
Support member applications with SSI, Medicaid, and other related applications by gathering relevant information in partnership with the member to complete and submit the application
Assist members in accessing transportation, by connecting to transportation benefits and providing transportation with Vanna when required to receive care or support
Adhere to documentation protocols for discharge planning, application enrollment, care coordination, and other documentation requirements related to responsibilities
Collaborate closely with the community team, including Team Leader, Vanna Coaches, Nurses, Housing Specialists, and Psychiatrists to support coordination of the member’s recovery
Provide feedback to Vanna technology and operations teams on existing processes and suggest improvements over time
Assist with employment, educational and voluntary efforts and other resources that improve the quality of life for the members
About You
Need to Have’s
1+ years in Case Management or Care Coordination
Experience working in mental health and/or primary care in and around the community of Philadelphia, PA
Belief that individuals suffering from serious mental illnesses can and will recover to achieve life goals when provided the opportunity, support, and community
Organizational skills to prioritize tasks and maintain a schedule
Are able to work in a team
Basic computer and phone skills and willingness to learn how to work with Google, Slack, and Vanna EHR tools
A valid driver’s license
Education Requirements
Bachelor Degrees in one of the following: Social Work, Psychology, Human Services, Behavior Health or related fields
Nice to Haves
Access to an operational and insured vehicle and willingness to use it to transport person served
Bilingual in Spanish and English to ensure we are meeting all members in their most comfortable language
