Care Coordinator

Job Description Job Description Summary of Position: Care Coordination is a service that will assist eligible individuals with providing the connective tissue between physical health and behavioral health. The care coordinator will work with different disciplines to ensure quality of care. The care coordinator will also work on projects with the nurse and medical assistant to review data and how physical health concerns may be affecting their behavioral health needs. This population includes; behavioral health or co-occurring issues, including children and adults, in gaining access through outreach efforts and linkage to specifically medical, but also social, educational, and other services. Care coordinators will work with the CPS, BCM, MHOP therapists, IOP counselors, and physical health team to ensure continuity. They will also work to ensure a smooth entry into the NET system of care. They will assist with tasks such as obtaining consents, releases of information, assisting in applying for MA and EVS verification. The administrative portion of this role will be assisting the team in report running in NET’s electronic health record system, working as a liason with the PCP and specialty providers
Essential Position Responsibilities: (Other duties as assigned)
• Must demonstrate genuine empathy and concern for individuals as indicated in our corporate vision, mission, and values statements

• Be sensitive to the needs of children, adolescents, and adults, particularly those that relate to family dynamics, and the educational and community structures
• Demonstrate an understanding of cultural competence and the principles of recovery
Minimum Qualification:
• Bachelor’s Degree required. Mental health discipline or related human services field (preferred)
• Experience working with children and adults with serious mental health or emotional
Disorders and substance use issues
• Valid Driver’s License (issued by state in which employee lives) and safe driving record
and proof of insurance coverage
· PA Child Abuse Clearance required
· PA Mandated Child Abuse Reporter Training Required
· FBI Clearance required
· PA Criminal Clearance required
· Health Assessment required
· TB Test required
