The mission of the Ambulatory Care Department is to deliver comprehensive, compassionate, and quality patient care. The mission is consistent with the hospital’s mission, which is to deliver a wide variety of community-oriented health services/programs. Some of the duties' assigned to the Patient Care Navigator is toprovide support and guidance to new patients (e.g., communicating required information before first appointments); Makes reminder calls to patients prior to their appointment. Follow-up with patients who missed clinic appointments and reschedule their appointment, prior authorizations for procedures, provide clinical support as neededand assist with building rapport with physicians and referral clinicians.
Requirements
QUALIFICATIONS:
Education Required - HS Diploma or Equivalence
License/Certification/Registration(s) Required - Graduate from an Approved Medical Assistant Program
Experience/Skills Required
Previous experience in a medical setting or 2 years relevant work experience preferred
Excellent communication/ interpersonal skills
Bilingual preferred
Knowledge of computer technology preferred. Successful completion of St. Barnabas Hospital orientation.
