Summary
Under the direction and support of the Assistant Clinical Director, or ACD, the Mental Health Specialist oversees a caseload of 5 to 10 veterans. Many of whom, are dealing with significant mental illness and substance use disorders. Workload includes (but is not limited to) ongoing clinical and nonclinical assessments, supportive counseling, community care access, logistical support (e.g., transport to and from medical appointments) and risk evaluation.
Responsibilities
Oversee and manage a caseload of 5 to 10 veterans.
Conduct one-on-one and group supportive counseling.
Provide intensive case management and community care access.
Work with onsite VA HUD-VASH behavioral health and medical providers.
Complete and manage daily, weekly, quarterly, and annual assessments (e.g., clinical and nonclinical progress notes).
Maintain and oversee onsite case management and programmatic files.
Help with programmatic compliance and meet with onsite auditors. Programs include (but are not limited to) VSSR, Fox Grant, HSH and COC.
Work with onsite and offsite staff to meet organizational deadlines.
Provide back-up coverage across six permanent supportive housing sites [and] programs.
Maintain confidentiality and HIPAA compliance.
Learn and manage electronic case management software and applications.
Requirements
Education BA/BS in psychology or social work preferred; however, life and work experience can work as a substitute. For example, experience with intensive case management [and] working with hard-to-serve populations.
Experience Minimum of two (2) years working with hard-to-serve populations and conducting intensive case management.
Experience conducting one-on-one and group counseling.
Familiarity with electronic case management applications and software.
Training or experience in de-escalation and risk assessment.
Connections with community care providers.
Qualifications A thorough understanding of hard-to-serve populations. This includes formerly homeless individuals, veterans, and those suffering from serious mental illness (SMI) and substance use disorders (SUD).
Understanding of confidentiality and HIPAA.
Familiarity with community resources and providers (e.g., SF VA).
Skills Excellent time management.
Punctuality, initiative and accountability.
Ability to think quickly and problem solve.
Comfort asking questions and learning on the job.
Familiarity and comfort using Microsoft Outlook, Word, and Excel.
Must be able to use onsite printers and scanners.
Good verbal and written communication skills.
Ability to conduct outreach and engage with clients.
Crisis intervention, de-escalation, and conflict resolution.
Meet clients where they are.
Perform duties in a respectful and professional manner. For example, with residents and veterans, staff members, and community partners.
