Job Description

Responsible for the daily management of residents assigned to caseloads, coordinate services and ensure quality care to the resident population, provide supervision to case managers. This position may have supervised / unsupervised contact and or conversational interaction with consumers while working in the residences, day treatments and habilitation facilities.

Essential Accountabilities:

Responsible for the daily operations of the residents on caseloads in the shelter.

Supervision of Part-Time Case Managers.

Provide direct care services to resident, including planning and implementation of goals and training, which will develop and enhance activities of daily living skills. These include but are not limited to:

a) budgeting

b) grooming

c) nutritional planning

d) use of public transportation/travel training

e) use of community resources/community integration

f) shopping/menu planning

g) housekeeping

h) sexual development/socialization skills

i) physical health maintenance

j) assertiveness/self advocacy training Monitor residents progress. Tasks essential to those duties include, but are not limited to: seeing, hearing and observing residents, performing manual tasks, reading and writing (documentation, lifting, climbing and descending stairs, traveling, boarding and de-boarding agency vehicles) and utilize public transportation (bus, subway) use of telephone to make and receive calls.

Responsible to coordinate evacuation and ensure residents safety in emergency situations (i.e., fires, fire drills). Tasks essential to these duties are the ability to exit in emergencies, which may require the ability to climb and descend fire escapes and assist others to do the same.

Responsible for all necessary written documentation including:

functional assessments; initialed and updated

medication updates

progress notes

service plans annual; quarterly updates annual

completion of progress reports and other assigned paperwork

Assist residents in planning and coordination of recreational/social activities. At times this may require escorting residents to these activities and/or participate in moderate physical activities as part of this task.

Responsible for the maintenance and updating of all resident files on a monthly basis.

Assist in training of Case Management staff.

Responsible for monitoring of the part-time counselors assigned to that area.

Attend all area Case Management meetings.

Responsible for reporting and documentation of all incidents to immediate supervisor and/or the Assistant Director of MHS within 24 hours.

Responsible for crisis intervention requiring 24 hours on call availability.

Participate in resident meetings and conduct all 30-day follow-ups on discharged residents.

Ensure that all counselor meeting and resident meetings take place on a timely basis.

Weekly supervision of assigned staff, to delegate tasks and provide professional direction and technical assistance.

Must be able to transport self to various sites.

Required to participate and attend meetings/trainings at various sites that require following different route of travel to worksite.

Able to lift/carry packages and boxes of supplies; as well as clients if necessary.

Ability to count, add, subtract, and manage money

Monitor environmental plant issues.

Communicate information with all staff and supervisor(s) verbally and in writing for the purpose of giving instructions, providing feedback and adjusting work practices.

If deemed necessary must be able to participate in these programs at various sites and varying times. The ability to complete the following training offered and perform the techniques taught in each area a required function of the position; AMAP, CPR and First Aid.

OTHER RESPONSIBILITIES: Participate in activities of other staff members in their absence or during periods of staff shortage.

Represent the agency at meetings, trainings not otherwise specified.

Ability to work flexible schedule as work schedule and locations are subject to change.

Drive facility vehicle when the need arises (escort, field trips, etc.).

This does not limit the assignment of duties or exclude the performance of other duties not mentioned, as assigned by supervisor.

Job Qualifications

Bachelor's Degree or Associates Degree with three years' experience in the Human Services Field. Supervisory experience preferred. New York State driver's license required.
