Shared Living Coordinator Shared Living Division

The Shared Living Coordinator position is 40 hours per week and reports to a Division Head. This position will be based out of our Main Office located in Waltham Ma, but will require frequent travel to the homes of Shared Living Providers homes, day programs, and other community locations. Hours are designed to meet the needs of the providers.
QUALIFICATIONS:
BA/BS in related field preferable with at least 2 years’ experience supporting people with Intellectual/Developmental Disabilities. In lieu of BA/BS, at least 5 years’ experience working in Human Services with experience supporting people with Intellectual/ Developmental Disabilities.
At least 2 years’ experience with DDS systems.
Knowledge of DDS regulations (2 years’ experience) and service planning process.
Excellent verbal and written communication skills
Ability to work independently and decisively.
Excellent organizational skills and ability to multi-task.
Cultural Awareness
Must hold a Driver’s license. Must have access to operational vehicle and be willing to transport Individuals.
RESPONSIBILITIES:
Create a pool of potential Providers with the assistance of the Recruiter and Social Media Coordinator to have Providers ready for potential referrals.
Participate in screening and interviewing of potential Shared Living Providers.
Set up Shared Living Provider Meetings to give opportunities for education and support to WCI’s Provider pool. Train Shared Living Providers in their responsibilities, including activities of daily living, community integration, money management, health care and medication management, data and documentation, and safety.
Complete all screening responsibilities for each Individual referred to the Shared Living Program. Meet with Individuals, Families and Guardians and DDS Service Coordinators.
Facilitate in all meetings during the matching process
Complete all required assessments needed before each placement.
Monitor and oversee the implementation and documentation of the Individual’s ISP in conjunction with DDS regulations
Complete all regulatory requirements for ISPs and Incident reporting in HCSIS
Establish and maintain all records for each Individual supported
Ensure and safeguard Individuals’ human dignity, rights, choices, and liberties, including 1:1 Training on Human Rights topics.
Schedule and complete home visits to provide support and ensure health and safety.
Conduct Individuals’ Satisfaction Interviews.
Coordinate of all services.
Assists with Individuals’ Benefit Management.
Monitor Individuals’ Funds per agency policy.
Monitor Individual’s Medication Administration.
Network with other providers and funders in the Shared Living community to ensure best practices.
Report significant issues, incidents, injuries (health, behavioral, psychiatric, etc.) to the appropriate Supervisor in a timely fashion; document issues, incidents and injuries in accordance with WCI’s Guidelines.
Participate in Support Coordinator AOD rotation for the Social Supports Division, including Shared Living
Crisis management as needed.
Maintain current Training in CPR, First Aid, Human Rights, Positive Behavior Supports, DPPC, Fire Safety, Standard Precautions, MAP and all required topics.
Perform all duties in accordance with WCI’S policies and procedures.
Any other task or responsibility assigned by the supervisor
