Hours:
Monday through Friday, 8:30 AM to 5:00 PM
Salary Range:
$25 to $51.49 per hour DOE
Interview Schedule:
Tuesdays: 4:00 PM to 6:00 PM
Thursdays: 4:00 PM to 6:00 PM
Additional availability: Mondays and some Thursdays mornings, typically around 12:00 PM or 12:30 PM
Job Description:
Overview:
The Scattered Site Case Manager plays a pivotal role in assisting clients in Santa Clara County to break the cycle of homelessness. This position primarily operates in the field, working closely with a caseload of individuals in various stages of homelessness, from street living to transitional and permanent housing. While predominantly field-based, the role requires occasional office hours for documentation and administrative tasks.
Responsibilities:
Provide individualized support to clients, employing a "whatever it takes" approach to address their needs.
Assist clients in accessing and maintaining necessary services including healthcare, social services, and interpersonal support systems.
Navigate clients through a coordinated entry system, ensuring they receive appropriate resources and assistance.
Develop personalized care plans for each client, focusing on strengths, barriers, health needs, income generation, and housing stability.
Identify and address areas where clients require assistance, such as scheduling appointments and applying for public benefits.
Emphasize housing and health as primary outcomes for client success, working collaboratively to achieve these goals.
Requirements:
Bachelor's degree in a related field preferred; a master's degree with 10 years of experience qualifies for the higher end of the salary range.
Previous experience working with individuals experiencing homelessness or in social services is highly desirable.
Must possess reliable transportation for field visits; personal vehicle will not be used for client transportation.
Strong interpersonal skills and the ability to work effectively with diverse populations.
Excellent organizational and time-management skills, with the flexibility to balance fieldwork and office duties.
Compassionate and empathetic attitude towards clients, with a commitment to supporting them in achieving stability and independence.
