This position will coordinate prenatal care services in accordance with obstetric clinical guidelines and recommendations leading to healthy pregnancies, successful birth outcomes, and a healthy environment for newborns. ReportabilityThis position will report directly to the Director of Social ServicesEssential Duties and ResponsibilitiesTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Care Coordination: Promote early entry into prenatal care, reduce missed scheduled prenatal appointments, and address any possible barriers to healthcare. Coordinate prenatal care services in accordance with obstetric care team clinical recommendations. Complete social screens and psychosocial assessments on all perinatal participants referred. Case management of perinatal participants, providing support throughout the prenatal and post-partum period. Connect perinatal participants to supportive services to address social-emotional needs and support healthy pregnancy and postpartum period, including transportation assistance and car seat safety.Support medical teams with perinatal participants identified with medical risk to connect them to specialized services to maximize successful birth outcomes. Support scheduling post-partum appointments. Documentation:Enter all screenings, assessments and encounter notes, and referral information in the electronic health record within 48hrs from date of service.Ensure all forms are completed accurately and submitted in a timely manner with appropriate supporting documents.Upload all participants' documents and completed forms into electronic health record.Draft grant program and progress reports as required by Director of Social Services.QualificationsThe requirements listed below are representative of the knowledge, skills, and/or ability required.Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.Knowledge, Skills & Abilities - Associate degree in Social Work, Family Studies, Human Services; or a minimum of 2 years of relevant experience in care coordination services.Experience working with perinatal populations and addressing needs related to prenatal and postpartum care.Demonstrate interest in perinatal health, public health, health education, or related field. Must possess knowledge of community resources, local service agencies, or local, state, or federal programs/services.Implement case management skills independently to include care coordination, referral follow-up, and case closure.Knowledge of community resources that can address social drivers of health among the perinatal population.A working knowledge and understanding of HIPAA and compliance with local, state, and federal regulations.Abide by grant report timelines and deadlines.Language Requirement - Must have the ability to communicate effectively in English and Spanish as used by participants.Physical Demands - Regularly required to sit; frequently required to reach with hands and arms, walk, stoop, kneel, crouch, talk or hear; must be able to lift objects up to twenty-five (25) pounds.
