Location: San Jose, California (Onsite)

Salary: Starting $25/hr. Depends on experience.

Job Type: Contract to hire.

Schedule: Monday-Friday 8AM-5PM

Case Manager purpose:

The Case Manager is responsible for outreach, engagement, and providing support and linkage to wellness/recovery resources, both in person and virtually to assist clients in reaching the optimal level of wellness and functional capability. The Case manager works with highest risk Medi-Cal members with the most complex medical and social needs, connecting with all their providers to coordinate areas of care and support.

Required Qualifications:

Bachelor’s degree in psychology or a related field, or equivalent training and experience that would provide the ability to perform the duties described above.

One year or more of case management or related experience.

Fluent in English and one of the Santa Clara Country threshold languages (Spanish, Tagalog, Vietnamese, Mandarin, Cantonese) preferred.

Trauma informed training preferred.

Preferred Qualifications:

Familiarity with community-based resources.

Excellent problem-solving and organizational skills.

Understanding of cultural issues, community resources and family dynamics.

Ability to communicate well with people of diverse cultural, professional, and experiential background.

Ability to establish and maintain effective work relationships as part of a multidisciplinary team.

Pay Details: $25.00 to $27.00 per hour
