What does an Outreach Case Manager?
The Case Manager will work with youth to create a solid foundation for their transition into adulthood through intensive case management services; housing stabilization; job coaching and access to supported employment and educational opportunities; resource procurement and advocacy. Under the supervision of the Program Director, the Case Manager is responsible for engaging participants in immediate needs assessment and access to resources as a part of a service delivery team. The Case Manager works within the limitations of the organization's resources and in compliance with grants, contracts, and agency policies and procedures. The ideal candidate will take initiative and be able to quickly and effectively build rapport with highly vulnerable and exploited youth; comfortable navigating systems; reliable and efficient; committed to the self-determination of marginalized young people; able to work collaboratively with colleagues and be steadfast in their goal of delivering impactful programming. Perform street based outreach, walking/traveling specified routes to locate and engage with at-risk youth offering supportive services and immediate resources
Support a warm, welcoming and inclusive environment at the drop-in center; assisting youth with basic needs, support services, and appropriate referrals
Know and implement elements of a strengths based approach, trauma informed care, harm-reduction, and positive youth development so that youth make informed decisions and build resiliency
Utilize program assessments and screening tools in collaboration with each youth to inform priorities of the individual service plan and make appropriate internal and external referrals for services
Support youth to enhance their education, vocation, relationships and communication, independent living skills, physical health, mental health, and more
Ensure client records and documentation is accurate, up-to-date, and confidential
Support the daily collection and tracking of statistical data to be used for completing programmatic required reports
Assist with housing navigation, search, and landlord recruitment efforts
Effectively work in a team-based care model to ensure youth receive well rounded, consistent, and coordinated support
Maintain working knowledge of local community resources and provide youth with referrals to other agencies for services when appropriate
Assist with presentations to other community members/agencies to increase awareness and community understanding of all branch programs
Attend weekly meetings to discuss program updates, report on cases, and identify service delivery needs
Demonstrate effective professional boundaries, self-care measures, and seek support from direct supervisors when in need of assistance, additional trainings, or accommodations
Comply with mandated reporting including timely verbal and written notice to Child Welfare Services and documentation in organization's risk management software
Maintain a controlled and safe environment for youth, assist with daily cleaning/sanitization and organization of work space areas
Report all accidents and unusual incidents based on the Channel Islands YMCA incident reporting process within the appropriate time frame to management team and assist with necessary documentation
Understand, follow and enforce all YMCA policies regarding safety, including emergency procedures and responding promptly and when necessary
Participate in all required federal, licensing, agency and/or contractually mandated trainings
May support other YFS departments with approval from supervisor

Qualifications Must be at least 21 years old
High School Diploma or equivalent required
College degree preferred
2 years related work experience
Complete required certifications within 30 days of hire
Complete all required Branch and Position specific trainings as required within two weeks of hire
Health Screening Report and negative TB test required prior to first day of employment
Ability to obtain a criminal record clearance
Ability to participate in DMV Pull Program
