About the Job:

The Gladney Home Case Manager provides clinical case management services in order to facilitate program goals and objectives using a wholistic approach for the Gladney Home residents. They work to identify each residents needs with a person-centered approach to address the development, writing, implementation, and follow-through of the comprehensive case management plan for each resident assigned to them.

Their general duties include managing case files, creating individualized adoption readiness plans, attending medical, specialist, and behavioral health appointments for residents, and helping guide the Gladney Home staff in the day-to-day situations involving residents. They are the primary point of contact for the DFPS/SSCC teams, all licensing professionals, schools, and YFTs. This position is also responsible for arranging assessments and other services throughout the year.

Responsibilities include:

Collects information, identifies individual needs, and develops a comprehensive case management plan.

Develops trusted relationships with Gladney home staff, counsels and guides them effectively care for residents and address behavioral concerns.

Develops and utilizes all agency and community resources available at the federal, state and local levels.

Arranges psychological, CANS assessments, and assessments for psychiatric treatment for residents.

Manages the residents schedules for counseling, equine therapy, and physical and occupational therapy.

Acts as the residents educational decision maker and is also responsible for enrolling the residents in school and attending any school conferences.

Problem solver who adapts well to changing needs, plans which may require evenings and weekends.

Local travel is required and may account for 30% of the time for 5-10 appointments per week depending on the number of residents.

Documents correspondences and monthly check ins with the residents, document any correspondence with Providers, DFPS, or SSCC teams and time spent with clients in the case file, case management systems.

Works with the residents and internal departments to prepare residents adoption readiness plan.

Maintains case file records and closes files in a timely manner on discharge.

An empathetic builder of relationships, meeting residents where they are.

Presenter and teacher of best practices to Gladney staff in the home.

Maintains client records, coordinating the collection of necessary information ensuring compliance with agency policy and regulatory agencies.

Maintains a thorough understanding of regulations, requirements and compliance and is the department contact for audits.

Work schedule will include minimum of three evenings a week until 7pm to engage clients after school.

Will be on a on call rotation for weekends and holidays to support staff and in the event of crisis management.

Minimum Requirements:

Bachelors degree in social work or related field is required.

2-5 years of experience with case management is preferred.

Experience in client assessment and adoption counseling a preferred.

Experience with TBRI is preferred.

Proficiency in Microsoft Office Word, PowerPoint, and Excel; Microsoft Teams.

Must be able to work a flexible schedule due to clients needs including weekends, evenings and nights.

Must be energized, motivated, and adaptable to meet changing needs in these dynamic situations.

Superb written and verbal communications skills.

Good organizational and time management skills.

Have a good understanding of scheduling.

Ability to deal with defiant behaviors, not easily offended.

Valid/current drivers license. Must be able to drive at night.

Must be physical able to carry and lift 30lb and preform de-escalation interventions.
