Description
RN Care Managers, in the acute care setting, provide a variety of services to appropriately help progress patients through the continuum of care. Key services include screening for post-acute needs; transition planning; care coordination; education; community care referrals; care planning in accordance with regulatory requirements; collaboration with the continuum care team; ongoing monitoring of plan effectiveness and reassessment of the need for Care Management (CM) services.

Required qualifications:
Bachelor's degree in Nursing or higher within 3 years of hire.
Upon hire: Oregon Registered Nurse License
2 years of Acute care hospital or community based experience.
1 year of experience with electronic medical records or documentation systems.
1 year of Home health, mental health, substance use, hospice, utilization review, discharge planning or care management experience
Preferred qualifications:
Bachelor's degree in nursing upon hire.
Upon hire: CCM certification

Pay Range: $49.77 - $73.13
The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.

How does Providence support you and your goals?
So Providence has been super supportive of my goals and all the different roles that I've had, from being a brand-new baby nurse to being in the management role. I kind of identified what my goals were, told my manager and they have gone full force into helping me achieve those. Most recently, the last couple years, I decided I wanted to get my master's degree. They told my manager that, she put me in contact with different schools and also told me about Providence scholarships. Helped give me the link, give me the information, helped me apply and I was able to interview and receive a scholarship to pay for my master's degree which was a huge weight off my back. Super supportive and helpful. And then she went and helped me through the entire process of getting my master's, supporting me, making sure I had what I needed to get my work done and get school work in and still have that work-life balance. And I found that even with people higher up within the organization reaching out to me about that. And it was amazing.
