Responsibilities
+ Provides clinical expertise in the nursing management of the patients, documentation, and care planning
+ Processes start of care workflow once start of care visit has been completed
+ Participates in IDG
+ Reviews visits plotted on SOC or Re-cert visit and adjusts as necessary
+ Maintains orders in HCHB
+ Manages admissions, deaths, discharges as well as assists with these to make sure proper documentation is completed
+ Assists in data collection for compliance, quality assurance, implementation of nursing policies and procedures, and staff development as assigned
+ Upholds effective communication with referral sources to ensure smooth transition of admissions
Qualifications
+ Associate degree in Nursing with current license
+ Three years practice as a Registered Nurse
+ Previous Hospice experience preferred
+ Knowledge of the Hospice Philosophy of Care
+ Ability to use Microsoft Word, Excel, Outlook, and other software programs
