**Shifts:** 8.30am - 8.00pm
**Salary: 12.44 - 13.04 per hour (higher rate for weekends)**

****Driving Licence Essential****
**NVQ/QCF Level 3 in Care is essential**
**Essential:**
* Due to the nature of this role, it is essential you either have an NVQ Level 2 or hold experience within care, particularly within challenging environments, whether this be paid, or personal.
* A manual driving licence and being comfortable driving a larger vehicle is essential.

Realise provides tailored support for people who may be living with their family, living in their own accommodation or to those who wish to progress to more independent living.
The Halcon Centre is a purpose-built residential facility for younger people with physical disabilities.
**As part of your role, your duties will include:**
* To undertake completion of full transition planning of new individuals and services into Realise.
* To undertake the transfer of new customers and support teams to specialist team leaders when transition into the service is complete.
* Maintaining an environment that is appropriate to the needs of those we support.
* To support staff to enable customers to understand and manage their behaviours that may challenge or harm themselves and others through positive behaviour planning..

**In order to be considered for the role, you will need to:**
* Be passionate, possess good communication, and organisational skills.
* Be able to work flexibly across varying shifts, including weekends.
* Experience of managing staff teams
* Be able to engage, listen and communicate in a variety of ways.
* You will also need to understand the complexity of working with challenging behaviours, whilst demonstrating resilience and adaptability.
