The Department of Health and Human Services is responsible for delivery of the County's public health and human services to meet the needs of our community's most vulnerable children, adults and seniors. The Department provides services through more than 120 programs at more than 20 locations. With over 2,000 employees, the Department of Health and Human Services (DHHS) is the largest County Department.

As a Department, we believe the health and wellness of the county can only be achieved by ensuring that every resident has fair opportunities to reach their fullest potential. DHHS provides critical services for residents who face a variety of obstacles in achieving their full potential. DHHS plays an essential role in building a healthy and strong community.

These positions may be Full-Time, Part-Time, TERM or Grant funded. Some positions require multilingual skills in the Spanish/English languages.

If you are interested in working for the Montgomery County Department of Health and Human Services, we invite qualified candidates to apply for our Community Health Nurse I/II (CHN) positions.

Programs listed below are not inclusive of all potential vacancies. We currently have positions working in the following Health and Human Services Programs:

A) Aging and Disability Services -Adult Protective Services -Nurse Assessment: Adult Evaluation and Review Services (AERS) provides evaluation and assistance to medically fragile, disabled, and vulnerable clients (pediatric and adult) who are at risk of institutionalization. AERS staff conduct comprehensive evaluations to identify available services to help individuals remain in the community, or in the least restrictive environment, while functioning at the highest possible level of independence and personal wellbeing. The evaluation may be conducted in the home setting or the individual's current location. Upon completion of the evaluation, the

multidisciplinary team develops a plan of care that identifies services needed to help the person remain at home or in the least restrictive environment.

B) Children, Youth and Family Services - Early Childhood Services / Infants and Toddlers :

Montgomery County Infants and Toddlers Program (MCITP) nurses work as part of a

multidisciplinary team to conduct initial evaluations of children referred for early intervention services. MCITP staff conduct comprehensive evaluations to identify the needs of the child and family and to identify which services are needed. The evaluation may be conducted in the home setting or another setting of the family's choosing Upon completion of the evaluation, the multidisciplinary team develops a plan of care, with the family, that identifies services needed. MCITP nurses may be a part of the child's care team if medical needs are identified during the initial evaluation.

C) Public Health Services - Mobile Health Clinic: The mission of the DHHS Mobile Health Clinic is to alleviate health disparities by traveling around Montgomery County providing integrated medical, dental, behavioral health, and human services to underserved and vulnerable populations and communities. The Community Health Nurse performs comprehensive, cultural and linguistic health assessments; provides preventive screenings and testing; plans, implements, and evaluates case coordination, management, and referrals; coordinates resources to address social determinants of health; and provides clinical insight into and assists with the development of protocols, processes, and procedures for safe and effective delivery of services. Flexible work hours totaling 40 per week, with a requirement to work one or two Saturday(s) and/or Sunday(s) monthly with advance notice.

D) Public Health Services - Babies Born Healthy (Parttime): Community Health Nurses in this position provide support to pregnant persons enrolled in the Babies Born Healthy Program. Babies Born Healthy serves Black and African American persons who are pregnant, enrolled in Medicaid and reside in a high-risk community in the down-county community of Montgomery County. The program's goal is to reduce the disparities in birth outcomes experienced by African American families by offering culturally congruent care, via home visits, education, resource assistance, peer support and group activities. The Community Health is responsible for providing the initial intake and screening for all new enrollees as well as on going follow-up for the cases at higher risk for poor outcomes.

E) Public Health Services - Childhood Lead Poisoning Prevention and Asthma Case Management Program (TERM): The Childhood Lead Poisoning Prevention and Asthma Case Management Program is a free home-visiting program for children receiving or eligible for Medical Assistance who have elevated blood lead levels, moderate to severe asthma, or both. Provides in home case management for children with lead poisoning and/or asthma. Provides education on asthma management and reduction of environmental hazards. Assess physical, emotional and social needs of enrolled clients, and provide educational support and outreach. Performs in home case management for children with lead/asthma and assesses homes for health and safety hazards. Coordinates with health care providers.

This position may be filled at the Community Health Nurse I, Grade 21 level ($76,977.05) or the Community Health Nurse II, Grade 24 level ($84,05418 - $92,595.97).

Minimum Qualifications

Community Health Nurse I

Experience: None.

Education: Graduation from a National League for Nursing accredited college or university or an equivalent program as determined by the Maryland Board of Examiners with a Bachelor's Degree in nursing.

Equivalency: An equivalent combination of education and experience may be substituted.

License: Possession of a current license to practice as a Registered Nurse in the State of Maryland* issued by the Maryland Board of Examiners is required at the time of appointment. If the license is temporary, a regular Maryland RN license must be obtained within ninety (90) days of appointment. Note: There will be no substitutions for this section.

Community Health Nurse II

Experience: Two (2) years of full-time equivalent post-licensure experience as a RN Nurse, one year of which must have been in nursing area of public health, community health, pediatrics, or maternal.

Education: Graduation from a National League for Nursing accredited college or university or an equivalent program as determined by the Maryland Board of Examiners with a Bachelor's Degree in Nursing. Equivalency: An equivalent combination of education and experience may be substituted.

License: Possession of a current license to practice as a Registered Nurse in the Maryland State, of Maryland issued by the Maryland Board of Examiners is required at the time of appointment. If the license is temporary, a regular Maryland RN license must be obtained within ninety (90) days of appointment.

The State of Maryland accepts as valid the current RN licensure from a candidate's primary state of residence if that state has an interstate compact with Maryland. The states having this interstate compact are Arizona, Arkansas, Colorado, Delaware, Idaho, Iowa, Kentucky, Maine, Mississippi, Missouri, Nebraska, New Hampshire, New Mexico, North Carolina, North Dakota, Rhode Island, South Carolina, South Dakota, Tennessee, Texas, Utah, Virginia, and Wisconsin. (This equivalency only applies to candidates whose primary residence is not Maryland). Any Registered Nurse (RN) or Licensed Practical Nurse (LPN) applying for licensure and living in a member state of the Multistate Licensure Compact must take the NCLEX exam in their home state. Preferred Criteria
