Project Manager IV, Stakeholder Communication

Description: Job Summary:This joint Population Health Management Initiative (PHMI) is a partnership between Kaiser Permanente (KP), the California Department of Health Care Services (DHCS), and Community Health Centers (CHCs) across the state will advance high quality of care and population health management for all safety net patients served by CHCs in PHMI, who are primarily Medi-Cal beneficiaries or uninsured. The PHMI leverages KPs expertise as a high-quality integrated healthcare system as well as the deep experience of CHCs and DHCS serving the Medi-Cal population and communities to achieve improved population health outcomes for the Medi-Cal program. This new level of commitment represents greater partnership between KP, the state, and CHCs to serve low-income populations in alignment with the KP mission. The PHMI Team works closely with DHCS and CHCs to develop provider focused solutions and tools- including people, process, and technology- that address core PHM capabilities, expand specialty access, and support social health practice. Key outcomes of the initiative include improving health outcomes, equity, and quality of care for Medi-Cal enrollees through targeted investments, robust data exchange, solution design and build, and implementation support focused on PHM capabilities, specialty access and social health.This role is to provide project management support for the technology platform development and implementation. Responsibilities include managing large-scale projects from initiation to close-out. Drives the work of project team members by leading the coordination of internal and/or external resources to drive completion of project deliverables. Proactively monitors and identifies project risks, issues, and trigger events within and across projects. Ensures the alignment, buy-in, and coordination of multiple project stakeholders and vendors. Assists with monitoring vendor performance levels and ensures service level agreements are met.
