BASIC FUNCTION:
Howard University's Preterm Birth Reduction Initiative is seeking a highly qualified Perinatal Care Coordinator to play a pivotal role in medical records coordination, data analysis, and patient engagement. This hybrid position involves meticulous attention to health data indicators, recruiting eligible pregnant patients for the Pregnancy Companion App, and facilitating group prenatal visits. The ideal candidate is detail-oriented, possesses a strong data background, and excels in direct patient engagement, primarily via phone.
SUPERVISORY ACCOUNTABILITY:
Involves no responsibility or authority for the direction of others.
PRINCIPLE ACCOUNTABILITIES:
Data Retrieval, Analysis and Reporting:
Systematically retrieve and analyze historical medical records and health data.
Ensure comprehensive review and analysis of data from previous years.
Compile and organize data for reporting to DC Health and other relevant stakeholders.
Collaborate with the Program Manager to maintain accurate and up-to-date historical records.
Patient Engagement:
Identify eligible pregnant patients through medical records and initiate outreach via phone.
Recruit eligible patients to sign up for the Pregnancy Companion App.
Assist in enrolling patients into group prenatal visits.
Group Prenatal Visits Coordination:
Efficiently schedule and organize logistics for group prenatal visits.
Act as the main point of contact for participants, addressing inquiries and ensuring a smooth experience.
Staff group prenatal visits, overseeing educational sessions and addressing participant needs.
Collaborate with the Program Manager to enhance scheduling and staffing processes.
Maintain open communication with patients, providing information and answering queries.
MINIMUM QUALIFICATIONS:
Bachelor's degree in a relevant field (health sciences, public health, etc.). Strong interest and experience in working with medical records and health data. Excellent communication skills, especially in engaging patients via phone. Detail-oriented with a passion for accuracy in data management. Ability to work independently and collaboratively in a hybrid work environment. Familiarity with prenatal care and group education is a plus.
