Description:

Our Recovery Services Peer Specialist empowers individuals on their path to heal from substance use. You'll collaborate with clients to define their recovery goals and connect them to essential resources, fostering a safe and inclusive space. Utilizing a harm reduction approach, you'll provide ongoing support and advocate for their needs, promoting well-being and positive change and define their unique recovery goals. Lived experience and cultural competency are valued as you build trusting relationships and bridge the gap to a holistic recovery model. Regularly scheduled supervision may be shared with other health centers and nearby institutions.

+ You will be forging connections with individuals in our OBAT program and engaging with them on their individualized and self-defined journeys of recovery.

+ As a mentor, accompany patients to appointments, events, and meetings, ensuring they navigate the healthcare and social service systems with confidence.

+ Provide appropriate recovery information, referrals, and linkages to help individual improve recovery and reduce risk behaviors, including but not limited to healthcare support services, social work, financial assistance/entitlements, child-care, housing, food, clothing, transportation, and other identified appropriate community resources.

+ Facilitate and/or deliver preventative care to members according the guidelines deemed appropriate by clinical team leaders.

+ Champion harm reduction strategies, ensuring access to vital services like naloxone and sterile syringes.

+ Foster a sense of belonging by connecting patients with recovery support groups and utilizing diverse engagement techniques such as motivational interviewing and positive behavioral support techniques.

+ Collaborate with colleagues to craft personalized wellness plans, addressing both substance use and potential mental health concerns.

+ Cultivate a culture of trust, empathy, and inclusivity using non-judgmental language to build meaningful relationships with individuals and their support network.

+ Stay well-informed of the latest health topics pertinent to our community and share knowledge to empower patients in their journey towards holistic wellness.

+ Act as a liaison with external facilities and community organizations, ensuring seamless continuity of care for our patients.

+ Document encounters in the electronic medical record.

+ Collect patient information and outcomes through state and grant required documentation; schedule and maintain tracking for all data collection appointments and deadlines, if applicable.

+ Ad here to the established standards of performance and conduct within the health system, ensuring respect for patient rights and privacy.

+ Base decisions on organizational standards, prioritizing patient rights and privacy considerations.

+ Perform other duties assigned or as necessary.

SKILLS, EXPERIENCE, AND EDUCATION REQUIREMENTS:

• A High School diploma or GED required• Bilingual in Spanish and English strongly preferred.• Lived experience with substance use disorders, with at least two-years sustained recovery.• Successful completion of a CCAR approved Recovery Coach Academy before hire, or the successful completion of a CCAR approved Recovery Coach Academy within 6 months of hire.• Prior completion of the Certified Addiction Recovery Coach (CARC) certification preferred but not required. CARC certification required within first 12 months of employment .• Strong advocacy skills along with knowledge of community-based services, resources, and local recovery community.• Demonstrated understanding of the multiple pathways approach to recovery from SUD (including Medication for Addiction Treatment as one) and willingness to embrace a patient-centered, patient-driven approach that recognizes an individual’s preferences and autonomy.• Empathic understanding of patients, including the ability to assess multicultural needs, sensitivity to ethnic, cultural, gender, and sexual orientation diversity, values, beliefs, and behaviors.• Proficiency in establishing working relationships with patients, their families, professionals, and the community, including the ability to collaborate with a multidisciplinary health care team.• Ability to work in a fast paced, sometimes unpredictable environment.• Demonstrated flexibility with client’s priorities, evolving needs, and goals.• Proficiency in Microsoft Office applications, e-mail, and the internet; requires the ability to research community resources independently and to access and enter data, document in EHR.

This role may require remote/telehealth work as well as back up to the Peer Specialist/Recovery Coach role in our South End location, as necessary.
