Hospice Nurse Case Manager - RN
We are seeking a Hospice Case Manager (RN) for a full-time opportunity with our team in Portland, Oregon!
The Position: The Hospice Case Manager will provide care to the terminally ill Hospice patient as needed as well as assistance and understanding to the family in the home care situation and in time of bereavement. The Hospice Case Manager is a member of the Hospice Team in providing Hospice care and we hope that they will have experience in nursing, with emphasis on Hospice education.
Compensation Package:
Up to $52 Per Hour (Based on Experience)

Requirements:
Registered Nurse Licensed in the State of Oregon
2+ Years of experience a Registered Nurse - Ideally in the hospice setting
Home Care Home Base Knowledge Preferred
Maintains a current CPR and BLS certification
Minimal Call Schedule (By Phone Only) - Flexible
Current and active Driver's License
Self-directed and able to work with minimal supervision.
Demonstrates excellent observation, problem-solving, verbal and written communication skills; nursing skills per competency checklist.
Responsibilities:
Monday through Friday 8 AM to 4:30 PM
Provide home hospice visits (approximately 4 per day) in the Portland Metro area.
Regularly assesses and reassesses the nursing needs of the Hospice patient
Provides dietary counseling
Provides Hospice nursing services, treatments, and preventative procedures
Initiates nursing procedures appropriate for the patient’s Hospice care and safety
Observe signs and symptoms and report to the physician and IDG members any unexpected changes in the patient’s physical or emotional condition
Teaches, supervises, and counsels the Hospice patient and family members about providing care for the patient
Develops and re-evaluates the patient/family care plan in conjunction with IDG to meet needs and maintain continuity of care
