Job Summary The Care Coordinator is responsible for the completion of clinical and administrative tasks in primary care to ensure that patients receive health care that is competent, caring, and meets with a high degree of satisfaction. The Care Coordinator is a key linkage person in our Care Management Program, who helps patients navigate between services and resources at the site.
