Position Responsibilities : The Mental Health Counselor works within an interdisciplinary team to provide behavioral health evaluation and initial intervention services to older adults in the North Shore and Merrimack Valley region of Massachusetts. The Mental Health Counselor works cooperatively with the Aging network to bridge identified gaps in services for older adults that may be impacting mental and emotional well-being and functional independence.

. Essential Functions: (Core duties or tasks that are fundamental and not marginal to the performance of the job) Conduct home visits to high-risk elders in AgeSpan catchment area with behavioral health issues.

Use evidence-based screening and intervention tools and practices to conduct assessment of an older adult's physical, social, emotional and environmental status to support development of a brief intervention plan aimed at symptom reduction and stabilization.

Arrange for crisis intervention services when necessary.

Collaborate with and refer to other agency departments to address the needs of at-risk consumers.

Refer to outside agencies and outpatient providers such as regional CBHC programs and primary care physicians for ongoing treatment and follow up.

Complete and maintain documentation related to intervention plan.

Attend and participate in team and general staff meetings and supervision as scheduled.

Participate in in-service training, courses and workshops to maintain professional expertise and develop additional skills.

Assist in the orientation and training of new staff members as needed.

Other duties as assigned.

Qualifications: Bachelor's degree in human services with 3-5 years of experience in social work or a related field required. Master's degree in social work, counseling or related field preferred. Experience working with older adults strongly preferred. Strong clinical assessment, documentation, and communication skills required. Valid Driver's License and reliable transportation required as field work is an essential component of the position.
