Are You a Caregiver Ready to Step Into a Leadership Role?

HomeWell Care Services is looking for a compassionate, organized, and dependable Care Coordinator to join our growing team serving individuals and families across Duval, Nassau, and Clay Counties. This is a part-time position with real full-time potential as we grow. You will work directly with our Agency Directors in a supportive, fast-moving environment where your work genuinely makes a difference every single day.

What You Will Do

Be the go-to person for caregivers and families, keeping communication clear and care consistent

Manage scheduling and step in to cover shifts in the field when needed

Keep client records and care plans current and accurate in WellSky

Support onboarding, compliance tracking, and day-to-day operations

Flag concerns to our supervising RN and Agency Directors and help find solutions fast

What We Are Looking For

Experience in home care or home health, whether as a caregiver, scheduler, or coordinator

Active HHA or CNA certification

Comfortable stepping into the field when the team needs you

Familiarity with WellSky is a plus but not required

Proficient in Microsoft 365 including Word, Outlook, and OneDrive

A warm, professional presence with strong people skills and genuine compassion

Organized, reliable, and able to juggle multiple priorities without losing focus

Valid Florida driver’s license and reliable transportation

Must pass a Level 2 background screening

This is not a dead-end position. As you grow with us the path leads directly into a full Care Manager role with greater responsibility and compensation to match. We are building something and we want the right people building it with us.
