Care Coordinator OpportunityAre you a caregiver ready to step into a leadership role? HomeWell Care Services is looking for a compassionate, organized, and dependable Care Coordinator to join our growing team serving individuals and families across Duval, Nassau, and Clay Counties. This is a part-time position with real full-time potential as we grow. You will work directly with our Agency Directors in a supportive, fast-moving environment where your work genuinely makes a difference every single day.What you will do:Be the go-to person for caregivers and families, keeping communication clear and care consistentManage scheduling and step in to cover shifts in the field when neededKeep client records and care plans current and accurate in WellSkySupport onboarding, compliance tracking, and day-to-day operationsFlag concerns to our supervising RN and Agency Directors and help find solutions fastWhat we are looking for:Experience in home care or home health, whether as a caregiver, scheduler, or coordinatorActive HHA or CNA certificationComfortable stepping into the field when the team needs youFamiliarity with WellSky is a plus but not requiredProficient in Microsoft 365 including Word, Outlook, and OneDriveA warm, professional presence with strong people skills and genuine compassionOrganized, reliable, and able to juggle multiple priorities without losing focusValid Florida driver's license and reliable transportationMust pass a Level 2 background screeningWhere this role goes:This is not a dead-end position.
