Compensation Range: $20/hr - $25/hr commensurate with experience

Overview:

In collaboration between Housing Works Inc. and the Mayor's Office of Criminal Justice (MOCJ), the Emergency Hotel Program will provide individuals involved in the criminal legal system experiencing housing insecurity, or chronic homelessness, with emergency housing placement and reentry support services. This housing program operates utilizing hotels across NYC boroughs and offers medical, behavior health, reentry support services, connection to additional health and wellness services and housing placement assistance to residents enrolled to promote positive reintegration to the five borough communities, reduce recidivism and build stronger, safer communities. Reporting to a Case Management Supervisor, the Case Manager will be primarily responsible for conducting psychosocial assessments with newly enrolled clients, co-developing treatment plans, assisting with obtaining and retrieving vital documents, assist with enrollment in benefits and entitlements and preparing or gathering clients documents needed for housing placement. The Case Manager will have experience working with individuals who are justice-involved, knowledgeable of NYC resources, knowledge of the criminal legal system and case management experience.

Responsibilities:

Manage a caseload of approximately 35-45 clients enrolled in the Emergency Hotel Program.

Conduct psychosocial assessments with all newly enrolled clients.

Co-develop Treatment Plans with clients assigned to caseload and review every 30 days.

Meet with clients regularly in accordance to program workflow and contract deliverables.

Assist clients with obtaining, or retrieving, vital documents such as a government issued IDs, social security cards, birth certificates, etc.

Assist clients with benefits and entitlements.

Assist with gathering all documentation and assessments needed for clients and Housing Specialists to apply for subsidized housing or other, more permanent housing.

Participate in case conference meetings, staff meetings and other agency meetings.

Connect clients to internal and external health, wellness or other support services.

Complete required data entry and program outcome tracking as required for contract compliance.

Submit daily, weekly, monthly and ad hoc reports in a timely manner.

Provide clients with appointment reminders as needed.

Promotes Housing Works' core values in day to day activities.

Performs other tasks as assigned by supervisor

Provide culturally competent services in accordance to Housing Works policies and practices.

Engages in advocacy efforts as outlined in Housing Works' and the Justice Initiatives Department policies.

Promotes Housing Works core values consistently.

Ability to work with clients with diverse clients with varying levels of service needs.

Works collaboratively with other team members within the Justice Initiatives Department and cross-agency.

Other functions that may be required with this position include: Attend and participate staff meetings and trainings as scheduled.

Responsiveness to emails, texts and phone calls.

Ability to adapt and successfully uphold job functions in a full remote workforce or hybrid in-person remote workforce environment.

Minimum Requirements Bachelor's Degree required in social services, psychology, social work or other related field.

The following are qualifications and requirements of the position: Experience providing case management or direct client service.

Experience working with criminal justice involved population, as well as substance use and mental health issues.

Strong written documentation and communication skills

Good problem-solving skills and ability to work autonomously

Microsoft Office Suite, particularly Excel, required

Ability to obtain and maintain DOC clearance.

Experience utilizing EMR systems.

The following additional competencies are viewed as important to success in this position: Preferably bi-lingual in Spanish.

Excellent written and oral communication skills.

Excellent or proficient technical skills to perform tasks related to Microsoft Office, email and case management systems.

Ability to work independently and collaboratively with the team.

Dependability and reliability to arrive and depart timely for scheduled shift.

Ability to lift 10 + lbs.
