Role description The SSI Case Management Social Work Specialist Supervisor oversees a team that includes social workers; and outreach worker(s) to assess CAAP clients to determine if they are eligible for Social Security Disability Benefits (SSI/SSDI). The team works closely with the Disability Consultation and Assessment Unit, physicians and psychologists, in determining the presence of medical and psychological disorders that are disabling. The team will assist clients that are assessed to meet SSA criteria to ensure that they qualify for these benefits. This will include, completing application materials, researching and collecting necessary medical records, reaching and collecting non-medical collateral information, submitting completed application materials to the SSA office, liaison with the SSA and DDS offices to ensure that they have received necessary information, and coordination to ensure the client is placed in payment status once a favorable decision on disability is rendered. The supervisor oversees the overall functioning of the unit. The supervisor ensures that assessments are of sufficient quality that accepted clients meet the criteria of disability, materials are completed, gathered and submitted within defined timelines, and that applications materials are comprehensive and well organized and are acceptable by SSA and DDS, In addition, the supervisor ensures that workloads are divided equitably among the staff members, the SSI case management staff work effectively with other HSA staff and with the staff of SSA, DDS, and other community partners, and comply with HSA administrative policies and procedures.

Essential Duties and Functions

According to Civil Service Commission Rule 109, the duties specified below are representative of the range of duties assigned to this job class and are not intended to be an inclusive list; may include additional duties as assigned.

In conjunction with the management team, provide ongoing oversight of the SSI Case Management program to ensure that it meets or exceeds project goals.

Supervise Senior Social Workers assigned to the SSI Case Management Team and Triage Primary Assessments.

Perform ongoing review quality control of case management activities.

Ensure that case managers' client assignments are equivalent and workloads are balanced.

Conduct ongoing quality reviews to ensure the accuracy of the findings.

Ensure that clients who are referred for case management are seen and assessed within established timelines.

Coordinate with DECU Clinicians to ensure clients referred to Case Management receive accurate assessments to determine whether clients are eligible for Social Security Benefits.

Coordinate with the physicians and psychologists, to ensure clients, who do not have current primary care or behavioral health providers, complete consultative exams with the staff of the Disability Evaluation and Consultation Unit.

Ensure that each application for Social Security are completed accurately and thoroughly and submitted within defined timelines.

Ensure that the case management staff develop and maintain strong working relationships with referral sources and other staff that work with the client include: PAES, CAAP Counseling Service, Triage, SSIP, Shelter Staff, and Staff of the HSA SRO Housing Program, SSA staff and analysts, and staff and analysts of DDS.

Coordinate and oversee outstations of Case Management Staff in CAAP Triage and PAES as well as in Homeless Shelters the DHS SRO Housing Subsidy Program.

Ensure case managers maintain accurate and up-to-date data entry in COSTS.

How to qualify 1. Experience :

Two (2) years of verifiable experience as a Licensed Marriage and Family Therapist (LMFT), Licensed Clinical Social Worker (LCSW), or Licensed Professional Clinical Counselor (LPCC); AND

2. License and Certification :

Possession of a valid license as a Licensed Clinical Social Worker (LCSW), issued by the California Board of Behavioral Sciences (BBS); OR

Possession of a valid license as a Licensed Marriage and Family Therapist (LMFT), issued by the California Board of Behavioral Sciences (BBS); OR

Possession of a valid license as a Licensed Professional Clinical Counselor (LPCC), issued by the California Board of Behavioral Sciences (BBS).

Special Condition

This position requires the following Special Condition:

1 year (12 months) of experience in the Health or Social Service field providing direct service to diverse populations.

SELECTION PROCEDURES

Depending on the number of applicants, the Department may establish and implement additional screening mechanisms to comparatively evaluate the qualifications of candidates. If this becomes necessary, only those applicants whose qualifications most closely meet the needs of the Department will be invited to continue in the selection process.
