Job Summary

Join our team of nationally and internationally recognized experts in advanced heart failure. We're the 8th largest heart transplant program in the United States.

The NP will support the advanced heart failure patients in an ambulatory setting across all clinic populations. Patients in the AHF CCC include: those who have undergone heart transplantation, those with mechanical circulatory support devices, those who are being evaluated or who are currently waitlisted for heart transplantation, and those being managed in the heart failure disease management clinic.

Flexibility to work at all UCSF Health locations and in varying schedules.

Required Qualifications

Minimum of five (5) years experience as a Nurse Practitioner with heart failure, heart transplant, cardiac TCU, MCS, or cardiac ICU level care experience in both inpatient and ambulatory settings.

Preferred Qualifications

Experience with Phoenix MCS and transplant EMR platform in Epic.

Heart Failure nurse certification.

Prior experience with policy and procedure development.

Prior experience as presenter at scientific conference on Cardiology or Advanced Heart Failure.

Experience with acute support mechanical circulatory support devices.

Pulmonary hypertension medication management (i.e., remodulin, prostacyclin, etc.).

Regulatory experience with transplant patient population management and requirements.

License/Certification

California driver's license (CDL) if required for position.

Current national certification from an accepted certifying agency and provide by first day of work. Accepted Certifications for this position:

Current and active RN and NP and Furnishing license in California.

Complete a BRN recognized graduate master's degree program as a nurse practitioner.

Active and/or transferable DEA license to California and provide evidence by 90th day of clinical practice. (It is the responsibility of the employee to transfer their DEA license to their new place of employment if from out of state, via online DEA portal and provide evidence of transfer).

Submit enrollment with CMS Medicare and Medicaid through credentialing by first clinical day. CCS enrollment as appropriate for pediatric environments.

Current AHA BLS.

State regulation requires that you have a supervising physician to practice.

Incumbents with less than 6 months of NP experience will be hired as a Nurse Practitioner 1.

UCSF Medical Center and Campus requires certain healthcare providers to be credentialed. In most cases this is done through its Office of Medical Affairs & Governance (OMAG) prior to the first working day. Credentialing and approval of the Standardized Procedures takes between 60-90 days but can vary.
