Clinical Account Executive- Hospice

Summary

The Clinical Account Executive coordinates the transition of hospice referrals to the agency beginning with the initial referral process. After initial referral, the Clinical Account Executive will continue to coordinate communication between the referral source, and the hospice agency to ensure a smooth transition. The Clinical Account Executive is also responsible for optimizing a growth strategy to increase hospice appropriate referrals through service delivery and education. The primary role of this position will be to serve referral sources in facilities, but may be asked to work with other referral sources as needed.

Responsibilities

\n Coordinates the referral and admission of patients to hospice from facilities

\n Coordinates education and services with facility case managers, physicians and palliative care teams as appropriate

\n Evaluates patient's eligibility and suitability upon request from the physician.

\n Receives and writes referrals from the physician and sends to the appropriate hospice location.

\n Acts as a liaison between the patient/caregiver and the physician and the hospice agency.

\n Coordinates the care of the patient between the physician, facility and hospice agency and updates the hospice agency regarding changes through a conference note.

\n Generates orders given by the physician and forwards to the hospice agency.

\n Communicates to the hospice agency any orders or changes that the physician implements.

\n Facilitates/orders DME equipment and sets up delivery

\n Facilitates discharge ensuring that Hospice staff are available to admit and DME, medications and medical supplies are available

\n Communicates with the hospice agency Executive Director regarding any complaints or concerns that the facility and/or physician has.

\n Manages any referrals to hospice from the facility and coordinates communication regarding the referral/admit

\n Responsible for executing the growth strategy of assigned facility accounts to increase company market share through relationship development and education.

\n Serves as an educational resource to physicians, nurses and other practitioners within the facility as it relates to hospice.

\n Attends the weekly Sales and Operations meetings of assigned agency

\n Responsible for achievement of admission goals/expectations.

\n Education and Experience \n Prior Hospice experience required.

\n Must be a licensed RN in the state of practice

\n Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
