Job Description Job Description Description: Coordinates patient/client care. Coordinates and integrates care provided by Agency staff, including contractual
providers.
Ensures communication with all physicians involved in a patient’s/client’s plan of care and other healthcare
practitioners.
Integrates orders from all relevant physicians involved into the plan of care and ensures the orders are approved by the
responsible physician.
Coordinates referrals.
Participates in activities relevant to professional services including development of qualifications. Recommends
resources needed to provide care. Makes patient/client and personnel assignments.
Assures that patient/client needs are continually assessed. Assures reassessment of the patient’s/client’s needs is
performed by the appropriate healthcare professional when there is a significant health status change in the
patient’s/client’s condition, at the physician’s request, and after hospital discharge.
Assures the development, implementation, and updates of the individualized plan of care in partnership with the
patient/client, representative (if any), and caregiver(s).
Ensures provision of services as ordered by the physician as indicated in the plan of care, including patient/client and
caregiver education and patient/client, caregiver, and family counseling. Prepares clinical notes as appropriate.
Assumes responsibility for continuity, quality, and safety of services delivered in compliance with state and federal regulations.
Requirements: Education: Graduate from an accredited diploma, associate, or baccalaureate nursing program. Must complete Texas HHSC’s
Computer Based Training for appropriate category(ies) of license.
Licensure: Active, unencumbered state license as a registered nurse, or a multi-state license issued by a Nurse Licensure
Compact (NLC) member state. Current driver’s license.
Experience:
Two years of
experience in Home Health care as a Registered Nurse . Management experience preferred.
Skills:
Nursing skills as defined as generally accepted standards of practice. Excellent interpersonal skills and the ability
to communicate effectively. Demonstrates proven decision making skills. If required to make visits, proof of
current CPR certification.
Transportation: Reliable transportation and valid auto liability insurance.
Environmental and Working Conditions:
Performs duties in an office environment. If required to make patient/client visits, may work in patients’/clients’ homes in various
conditions; possible exposure to blood, bodily fluids, and infectious diseases; ability to work a flexible schedule; ability to travel
locally; some exposure to unpleasant weather; PRN emergency calls. Supervising Nurse or Alternate must be available at all times
either in person or via telecommunications.
Physical and Mental Effort:
Prolonged sitting required, with the ability to operate office equipment which may include heavy lifting, bending, and standing.
Requires the ability to handle stressful situations in a calm and courteous manner at all times. If required to make patient/client
visits, prolonged standing and walking required, with ability to lift up to fifty pounds and move patients/clients. Requires working
under some stressful conditions to meet deadlines and patient/client needs, and to make quick decisions and resource acquisition.
