Provides pharmacy services consistent with evidence-based medicine. Provides excellent service to our customers, which include patients, medical staff, and nurses.
Oversees all aspects of pharmacy operations, ensuring accurate prescribing, dispensing, and administering medications throughout the hospital.
Interprets Physician orders and provides advice directly to the ordering provider.
Evaluates the patient's drug therapy. Clarifies dosage, drug interactions, allergies, contraindications, and therapeutic duplication with the prescriber.
Performs renal and hepatic dosing, IV to PO conversion, aminoglycoside and vancomycin dosing, anticoagulation monitoring, antibiotic selection and dosing, and pediatric weight-based dosing
Accurately enters and verifies medication orders using the Meditech electronic medical record.
Discourages the use of patient's own and non-formulary medications. Discusses formulary alternatives with the prescriber.
Monitors and investigates adverse drug reactions and medication occurrence reports.
Monitors lab values and suggests dosing alternatives.
Documents interventions with prescribers
Evaluate chemotherapy regimens, verifying against diagnosis.
Actively participates in patient rounding and interdisciplinary committees.
Practices opioid stewardship to provide proper pain control
Supervises technicians, providing checks and guidance for their work process.
Ensures all IV admixtures are prepared using aseptic technique
Ensures all medications are accurately prepared, labeled, and checked before leaving the pharmacy.
Participates in the Pharmacy Improvement Process.
Functions as a preceptor to new employees and students.
Contributes to Departmental cost-saving initiatives. Ensures all chemotherapy preparations are checked by one Pharmacist and two other Pharmacy personnel.
Maintains the security and perpetual inventory of controlled substances.
Assists with the training of new staff/students.
Participates in the control, use, and monitoring of investigational drugs.
Provides evidence-based drug information and teaching to our patients and customers.
Practices Pharmacy following State and Federal statutes/laws.
Completes all mandatory educational assignments on time.
Qualifications
Educational / Minimum Requirements:
Currently enrolled in accredited Pharmacy School. Consideration will be given to candidates with demonstrated excellent clinical skills with the approval of the Pharmacy Manager.
State/Federal Mandated Licensure or Certification Requirements:
Eligible for a State of Connecticut License as a Registered Pharmacist in good standing once matriculated.
Bristol Hospital Mandated Educational Requirements:
General orientation at time of hire. Healthstream and Communicating with Deaf and Hearing Impaired . Any additional educational requirements as determined by the Pharmacy, Education or Human Resources Department.
Special Requirements:
Computer skills as related to pharmacy operations.
Physical Requirements:
On feet for extended periods. Periodic stooping, bending, and walking.
Work Environment:
Main Pharmacy and patient care areas. Ability to work in a variable environment that will include a rapid pace, frequently changing conditions, and work related distractions. Moderate exposure to hazardous substances found in the Pharmacy.
Cognitive Requirements:
Strong clinical and critical thinking skills. Good communication skills, both written and verbal. Ability to adapt to constant changes and thrive.
