Responsibilities of the Supervisor of Case Management:
This is a fantastic leadership opportunity for a talented LSW/LCSW/RN who is looking to expand their career. This position is 50% administrative and 50% clinical. It includes managing a 50% caseload of acute inpatient rehabilitation patients and 50% is towards various administrative responsibilities to ensure the clinical care delivered by our staff meets or exceeds our standards of excellence.

Performs case management and coordination of care by using a systematic approach for evaluation and discharge planning to ensure quality services and safe discharge
Responsible for the development and implementation of a patient specific, safe and timely discharge plan and corresponding documentation
Builds relationships and coordinates with payer sources to assure proper reimbursement for hospital-provided services
Responsible for the development of case management talent, including enhancing workflows and increasing the level of clinical expertise of staff
Assists with interviewing, selecting, training and validation of new staff
Collaborating with other team members including nursing, medical staff, therapies, administration, utilization review and the business development team
Participates in quality improvement projects and sites on hospital wide quality improvement committees as needed
Ensure adherence to State, Federal, Local and SHS policies and procedures
MINIMUM EDUCATION : Bachelor's degree required (Master's preferred).
MINIMUM WORK EXPERIENCE: A minimum of 2 years of related experience (preferably as a Case Manager or in a health care setting)
