**Job Summary and Qualifications**

Plans, organizes, controls, and supervises activities in hospital pharmacy according to hospital policies, standards of practice of the profession, and State and Federal laws. Recommends innovations in the practice and function of the hospital pharmacy to Administration and carries out mutually agreed upon programs.

**Major Responsibilities:**

+ Develops and enforces policies and procedures that promote cost-effective, appropriate, and safe medication use.

+ Develops and maintains a medical staff-approved formulary.

+ Supervises medication storage and preparation areas throughout the facility and off-site clinics.

+ Provides for the educational needs of health care professionals, patients, and their families.

+ Ensures maintenance of an adequate medication supply. Ensured the integrity of the medication supply.

+ Establishes specifications for the procurement of medications, chemicals, and biologicals.

**Education & Experience:**

+ B.S. or PharmD from an ACPE-accredited School of Pharmacy - Required

+ Completion of a 1-year pharmacy practice residency, specialty residency, or fellowship, or at least 3 years of pharmacy experience is strongly preferred - Preferred

+ Experience using computer applications including word processing, spreadsheets, and pharmacy information systems required – Required

**Licenses, Certifications, & Training:**

+ Current Pharmacist license in good standing with the appropriate State Board of Pharmacy - Required
