THIS POSITION IS RECRUITING FOR 1 POSITION WORKING IN ADA, OK AREA.

Position Summary:

The Health Access Network (HAN) at OSU-Center of Health Sciences, in partnership with the Oklahoma Healthcare Authority, is working to improve access, quality, and continuity of care to SoonerCare members in the State of Oklahoma. The RN Case Manager offers case management/care coordination to persons with complex health care needs and contributes to the five foundational services in the Health Access Network: facilitating Members' access to all levels of care utilizing case management/care coordination services, telemedicine services, electronic health records, improved access to specialty care services and expanded quality improvement strategies.

Environmental Hazards:

Possible exposure to hazardous material, hot and cold, blood borne pathogens.

Physical Requirements:

Ability to lift and carry 8-10 pounds, stoop, reach, stand, walk, finger, grasp, feel, talk, hear, see, and perform repetitive motions with or without reasonable accommodations.

Education & Experience

Position Qualifications:

Required:

-Associate's degree in Nursing plus 2 years nursing experience OR, Bachelor's degree in Nursing plus 1 year of experience as an RN OR, Master's degree in Nursing OR, Master degree in Social Work plus 1 year of experience as an LCSW.

-Current & active RN, or LCSW licensure issued by Oklahoma State Boards of Nursing or Social Work.

-Valid Driver License.

-Case management/outpatient clinic experience.

-On Dec. 7, 2021, a federal judge temporarily blocked the implementation of the federal contractor vaccination mandate and its associated guidance. Following the injunction, Oklahoma State University has, consistent with applicable law, paused the vaccination mandate.

Skills, Proficiencies, Knowledge:

Knowledge and skills in chronic disease management. Knowledge of national, state and local health and social service provider agencies; exceptional interpersonal communication skills - to include writing/documentation skills. Critical thinking skills and sound judgment; exhibit initiative and ability to work independently and within a team; ability to meet deadlines and prioritize workload; exhibit cultural sensitivity and the ability to work with diverse patient population and professional staff, faculty and medical students within academic health care setting. Computer skills.

Preferred:

-A minimum of 3 years clinical practice experience

-Case management certification (or eligible to obtain).
