About the Role
To support U.S. Urology Partners by facilitating care in all phases of Benign Prostatic Hyperplasia (BPH), including diagnosis, treatment, and transitional care continuum. Effective nurse navigators build working relationships, solve problems, direct patients to resources and manage information. What You'll Be Doing
Coordinate patient care including scheduling, care transitions, communication, and referrals for services in collaboration with physicians and the broader care delivery team across the continuum of care
Assess patients' current and future needs across a broad spectrum of medical, psychosocial, and financial aspects and make referrals to internal and external resources as appropriate
Interface with other healthcare teams for appropriate referrals and services
Provides and reinforces education to patients, families, and caregivers about diagnosis, treatment options, side effect management, and post- treatment care and survivorship
Provides education and facilitates coordination on insurance appeals and funding for oncology treatments
Establish and maintain relationships with patients, family and caregivers throughout the continuum of care
Promotion and facilitation of support groups and services offered by U.S. Urology Partners
Promotes awareness of clinical trails to patients, families, and caregivers
Ensures documentation of patient encounters and provided services
Facilitate language translation or interpretation services
Build partnerships with local agencies and groups for referrals
Document all patient coordination activities, education, and other interactions in designated inpatient and outpatient electronic health record systems
Willing to travel to other locations for business needs
Maintains strictest confidentiality
Performs related work as required
What We Expect from You LPN, CMA. LPN Preferred
Minimum two (2) years of experience working in a medical practice
General knowledge of healthcare insurance system
Excellent verbal and written communication skills
Electronic Medical Records Experience
Experience in relationship building with people in crisis
Urology experience
Reasoning Ability
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Computer Skills
To perform this job successfully, an individual should have thorough knowledge in computer information systems.
