Manager Care Management in CASE MANAGEMENT Unit at Providence St. Mary Medical Center

The Manager of Case Management is responsible for the daily management and organization of all activities and staff within the Case Management Department including case management, discharge planning, and utilization management. The Manager acts as a liaison between patients and families, department staff, physicians and the healthcare team, community agencies, managed care representatives, skilled nursing facilities, and community agencies. The Manager is responsible for establishing, implementing and monitoring best practice and established clinical standards to assure the quality of patient care. Additionally, the Manager will participate in organizational initiatives and communicate regularly with other stakeholders to lead improvement in utilization of resources.

Required qualifications:

Bachelor's Degree Accredited College of Nursing

Upon hire: Washington Registered Nurse License

6 years Case Management Experience in a Complex Hospital System

4 years Leadership experience

Preferred qualifications:

Master's Degree Social Work from an accredited school

Upon hire: ACM or CCM Certification
