Job Description

SUMMARY OF DUTIES AND RESPONSIBILITIES:

Reporting to the Senior Director of Quality Assurance and Performance Improvement, the Performance Improvement Advisor role provides technical assistance, coordination, and management of key quality improvement initiatives across select NYC Health + Hospitals/Gotham Health sites.

The Performance Improvement Advisor helps develop and guide performance improvement initiatives in ambulatory care that align with strategic priorities. Understanding quality improvement strategies, project design and development methods, data aggregation, analysis and reporting techniques are crucial skills, plus the ability to work well with busy clinical teams. The ideal candidate will work synergistically with colleagues to drive specific quality improvement initiatives to successful completion and long-term sustainment

General tasks and responsibilities will include:

Coach performance improvement (PI) activities across Gotham Health clinical sites that align with the Gotham Health performance improvement plan, data-driven clinical site priorities and the NYC Health + Hospitals/Office of Quality five (5) strategic pillars: Quality & Outcomes, Culture of Safety, Financial Sustainability, Care Experience and Access to Care;

Assist facility leaders with selecting project metrics, utilizing baseline data to identify high priority areas for improvement, establishing a diverse, functional team with clearly identified roles and responsibilities and developing meeting schedules;

Engage frontline healthcare workers in the process of identifying value-added opportunities for change;

Under the supervision of the Senior Director of Quality Assurance and Performance Improvement, promote understanding and execution of all phases of the PI life cycle at clinical sites including: drafting aim statements, scoping and aligning projects to strategic goals, developing appropriate tests of change, selecting and monitoring key metrics (such as process, balancing, and outcome measures), coordinating data oversight activities and reporting measurable results to staff across all levels of the organization;

Provide individual and/or team-based educational training on the appropriate utilization of PI frameworks, such as PDSA cycles, Institute for Healthcare Improvement model for change, and other improvement concepts;

Guide teams in the use of problem-solving and analytical tools (e.g. Fishbone/Ishikawa Cause & Effect Diagram, Driver Diagram, Run Charts, Control Charts etc.) to enhance project planning, implementation, reporting and system-wide learning and performance evaluation;

Offer guidance to teams developing PDSA action plans, support for completing required standardized templates to outline project goals, deliverables, deadlines etc., and assistance in creation of measure charts to display key performance metrics

Attend facility and corporate-based PI forums including, but not limited to, Facility-Based Collaboration Councils (FBCCs) and site Performance Improvement committee meetings

Utilize proven methods to minimize and resolve conflicts among individuals and teams, while encouraging collaboration to improve the likelihood of projects successfully progressing and undergoing continuous improvement

Guide internal staff through the process of collecting and analyzing data for PI purposes

Perform other duties as indicated by leadership

Minimum Qualifications

* Qualification for Central Office.

# Qualification for Health Care Facility.

1. a. A Baccalaureate Degree in Public Administration, Public Health or Health Care Specialization; and,

b. Five* years or four# years of increasingly responsible experience in hospital administration, with particular emphasis on development and evaluation of health care delivery services; or,

2. a. A Masters Degree in Public Administration, Public Health or Health Care Specialization; and,

b. Four* years or three# years of increasingly responsible experience in hospital administration, with particular emphasis on development and evaluation of health care delivery services.

Department Preferences

EDUCATIONAL LEVEL:

Minimum of Bachelor's level degree with preference for Master's level degree in healthcare administration, business, public health, allied or related clinical field; preference for a clinical background, particularly nursing

KNOWLEDGEABLE IN :

Minimum of 2 years healthcare experience, preferably with experience in the ambulatory care/primary care setting

YEARS OF EXPERIENCE:

Minimum of 2 years healthcare experience, preferably with experience in the ambulatory care/primary care setting

PREFERRED SKILLS: Strong preference for candidates trained in Model for Improvement, Lean, and/or other quality improvement methodologies

Ability to proficiently utilize Microsoft Word, Excel, and PowerPoint programs plus have data charting skills

Willing to travel to different Gotham Health facilities

Highly motivated, self-starter with strong leadership, change management, planning, organizational and project management skills

Applicant must possess excellent verbal, written, leadership and facilitation skills along with a sound understanding of business principles, operations and process improvement strategies

Excellent interpersonal skills and ability to manage a multitude of activities in a fast-paced environment

Understanding of HIPPA and other regulations designed to protect patient rights and confidentiality

Commitment to maintaining fluency in current and innovative PI theory and processes
