The Case Manager will provide client-centered case management services to gay, bisexual, MSM (men who have sex with men), and male-to-female (MTF) transgender individuals who are at risk of or living with HIV/AIDS in Dallas County. The Case Manager assists clients with remaining connected to appropriate medical, behavioral health, substance abuse, and other ancillary services to stabilize their health, meet their needs, and avoid higher levels of care.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Assess clients needs, situations, and support networks to enable them to improve and/or maintain their current health situation and achieve their short- and long-term goals.

Develop Individualized Health Plans to improve clients overall health and well-being, especially people living with or at significant risk for HIV/AIDS.

Work collaboratively with medical personnel, as applicable, to ensure adherence to prescribed therapies.

Maintain confidential up-to-date client contact and fulfillment of treatment goals and help clients adjust to changes and challenges in their lives such as medical diagnosis or illness, divorce, or unemployment.

Provide PrEP, Hepatitis, and HIV education; Provide all linkage to care services for STIs, PrEP, HIV, nPEP, and Hepatitis infection.

Advocate for and refer clients to community resources (Medical Case Management, emergency food assistance, PrEP) that will improve clients' overall health and well-being.

Respond to crises and be knowledgeable of available community resources and/or evidence-based treatments.

Work to build the trust and confidence of clients to overcome any fears or anxiety related to their health condition(s) or with navigating the healthcare system.

Assess clients essential social support needs, life situations, and support networks to understand how these factors may impact their engagement in medical care.

Work closely with internal and external partners to implement Individualized Treatment Plans for clients and to identify services to address other aspects of their lives, such as mental health and substance abuse treatment, domestic violence counseling, housing programs, and education and employment services.

Follow-up with clients to ensure they attend their appointments, fill their prescriptions, take their medication(s) as prescribed, and/or address any barriers or challenges to adhere to their medication(s), and remain engaged in care, among other things.

Perform other duties as assigned.

QUALIFICATIONS:

Bachelors degree in a social science or related field from an accredited college or university is required and 2 years of work experience serving sexual minorities of color.

Good working knowledge of federal, state, and in-house data management systems

Good written and oral communication skills

Knowledge of referral sources for client needs

Relationship with colleagues at partnering and collaborative agencies

Adept at keyboarding and organization, as well as filing and coding data

Exceptional verbal and written communication skills

An aptitude and preference for detail attentiveness, organization, and order
