About the Program

Graham Windham’s School Based Early Support programs located both in Manhattan and the Bronx offer family support and empowerment, case management services to partner with families to keep them together while ensuring that children will be safely cared for. We partner closely with schools to connect neighborhood families to an array of supports and services. Our strategy for working in schools is to embed ourselves in the school and community, and create strong collaborative partnerships with Administrators, teachers, parents, students, and other community organizations. We work to support families so that children and adolescents are safe and able to thrive

in stable, nurturing homes and communities.

Position Overview

To assist the unit’s team in efforts to develop and maintain a well-organized, efficiently operated office which provides a high quality of services to the children, their families and the communities in which they live.

Duties and Responsibilities (but not limited to):

+ Manage phone messages, handling correspondence, ordering supplies, and performing other essential day to day activities, which include but are not limited to maintaining sign in/ out sheet; updating program’s office calendar; tracking staff schedules; establish and maintain an administrative filing system within the office; oversee records management, including off-site storage; create and maintain case records; process new referrals and create intake packets.

+ Case Aides will follow up on unit case work standard contacts, inclusive of completion of weekly cross-checking grids for each unit in the program.

+ Case Aides will assist with collaterals contacts and documentation of the contacts made with families and service providers.

+ Support Program Director, Program Supervisors and School Liaison in ensuring that PROMIS and CONNECTIONS systems are updated regularly .

+ Maintain a clearinghouse of all forms and resources being used by Graham Windham Preventive Services. Updating Shared Drive.

+ Collecting information regarding community resources and disseminating said information to staff and client families.

+ Responsible for the development of various types of logs and systems in order to monitor program activities and outcomes

+ Update and maintain client roster, assist with FASP due date list, and administrative binders.

+ Collect educational data (including report cards and attendance records) and maintaining client roster.

+ Provide administrative support to Program Director, Program Supervisor, and School Liaison to ensure that program is in compliance with OCFS and NYC Children’s Services’ policies and protocols.

+ Provide training and technical assistance to program staff in the areas of clerical and administrative function and to provide clerical support on emergency situations. Orient new staff and interns.

+ Enroll staff and interns in trainings and workshops using STARS and other systems

+ Monitor the submission of and follow-up regarding time sheets , travel reimbursements, furniture requests, purchase orders, check requests and other correspondences to other agency departments. Update personnel who made request to detail status.

+ Taking minutes at staff meetings.

+ Ensure that all office equipment is functional at all times and follow up with repairs when necessary.

+ Support program staff by making casework contacts, including covering home visits and other collateral contacts when necessary. Cover home visits when program is understaffedto aid inassessing the needs of families.

+ Make school visits to help collect educational data

+ Manages mail and correspondence including family planning letters.

+ Has the ability to drive with clean driver’s license, can be asked to travel in and out of state when needed. (Preferred).

+ Case Aides are required to become fully trained and work on expertise in one of the following areas: motivational interviewing, Educational resources for families, HRA resources, Housing, Recreational services, Substance abuse resources & support programs, Health/ medical resources, and Care coordination resources.

+ Build relationships and support parents in coping with the trauma of child welfare involvement, making decisions for their families and communicating effectively with those involved in their case.

+ Partner with case planners to help families engage and navigate targeted resources

+ Invite parents, encourage parents to invite supports to conferences including Collaborative Case Planning , ERC, Safety Conference, Service Termination Conference, Six Month Service Plan Review, Joint home visits, Administrative Meeting with Families as well as 30 / 45-day conferences, Six Month Service) and court proceedings. Follow up on action tasks completed with case planners.

+ Encourage parents to engage in supportive interventions that will strengthen their families.

+ Cofacilitate and collaborate with school liaison, and community pathway worker in bi-weekly/monthly support groups utilizing Triple P, Network and Parent Café Model, (staff to be trained in the model).

+ Escort parents to HRA, scheduled court appearances for support, to scheduled appointments or shopping to purchase items requested by the agency.

+ Maintain and document interactions with parents within the databases (PROMIS and Connections).

+ Submit monthly tracker to supervisor regarding contacts made with parents for CP and case aides in a timely manner as well as CP contacts.

+ Complete other tasks and projects as assigned.

Full Time Position: Starting Salary from $42,172 - $44,172/annually + Potential to Earn Merit Performance Based Incentive for Eligible Staff + Comprehensive Benefits + Professional Development Opportunities. Qualifications

Qualifications:

+ High School diploma required

+ Bilingual English/Spanish/French/Haitian Creole preferred

+ Computer literate and working knowledge of MS Office and similar programs and use of office equipment

+ Willingness and ability to drive a car or van within the 5 boroughs of New York City, Long Island, and counties North of the city.

+ Ability to organize, manage and teach others in various Administrative and Clerical functions Effectively work in a team setting

+ Excellent communication skills, verbal & written

+ A strong interest in the enhancement of the quality of life for those who depend upon agency’s services
