Job Description Job Description Position Summary : Community Health Workers (CHW)s, who are based in local community-based organizations (CBO)s and who work as members of health care teams, empower "rising risk" patients and their caregivers to improve health and well-being through the delivery of culturally sensitive, peer-based education and support. CHWs report to CBO Supervisors and their day-to-day work is monitored and supported by CCHN Program Leaders.
Minimum Qualifications
Two years of CHW experience or comparable experience
Experience facilitating workshops and providing one on one health support
Comfortable conducting home visits
Strong interpersonal skills demonstrative of empathy and support
Willingness to visit with families in hospital and in the community
Strong written and verbal communication skills
Proficiency in Microsoft Office
Bilingual Required: English/Spanish or English/Cantonese or English/French Creole or English/Other Language
Responsibilities
Meets all position requirements as outlined in Scope of Work
Enrolls and manage caseload of patients who meet rising risk program criteria
Helps patients set and achieve program goals
Supports patients to navigate the health care system
Connects patients to social resources based on identified needs
Helps patients enroll onto/navigate patient portal
Conducts at least 2 home visits and provides appointment accompaniment as needed
Delivers practice-based education and support (101)
Completes required documentation associated with assessments and educational sessions using approved collection method
Submits all required documentation by designated deadline
Facilitate at least 2 community-based workshops per year
Attend all required staff meetings and trainings.
Compensation : $24.73 per hour
Hours : Monday to Friday, 35 hours per week
