Job Description Job Description Be a part of driving healthy change in our communitiesAbout the Position Responsible for navigation, outreach, and health promotion within Legacy clinics and neighborhoods located near clinic to increase awareness of cancers screenings and health promotion as well as adult medicine, OBGYN, dental, and behavioral health services. Collaborates with department staff to facilitate efficient utilization of resources which include referrals, linkage to care, communication with providers, access to community resources, and transportation.
Key Responsibilities Serve as an initial point of contact for patients, employees, clients, medical staff, and other callers seeking help from the department.
Assist patients in navigating the health services system. Coordinate needed physician referrals and appointments for patients.
Coordinate screenings and facilitate patient access to agencies and other community resources and transportation.
Place reminder calls to indicated patients and document outreach attempts in the patient’s chart/EHR.
Updates patient information in patients’ chart/EHR, as needed/appropriate.
Provide resolutions to the patient’s concerns within job scope.

Requirements High school diploma or high school equivalency (GED) is required.
Completion of approved DSHS-certified 160-hour competency-based Community Health Worker training program is required.
Certification as Community Health Worker is required.
Three (3) years of healthcare (physician’s office, acute care, long term care and/or case management) experience is required.
Experience in overcoming the health care system barriers and facilitate timely access to quality medical care is required.
Must be bilingual: Proficiency in speaking, reading, and writing the English and Spanish languages are required to perform the essential roles of this job.
