Job Type : Full-Time, Day-shift, Monday-Friday

Pay Rate: Starting at $19 hourly depending on experience

Summary : We are recruiting for a dynamic Care Coordinator who can provide consistent quality resident care and service; delivered throughout all shifts. The incumbent is responsible for scheduling organizes and managing all aspects of a patient's healthcare maintenance and treatment, including accepting management responsibilities of the Nursing Department in absence of the director of resident services.

Duties and Responsibilities include the following. Other duties may be assigned.

Demonstrates excellent decision-making skills and can understand the impact of those decisions.*

Sets and manages priorities.*

Accepts calls from staff during nights and weekend.*

Conducts meetings/training as required/requested.*

Communicates in a clear, accurate and timely manner.*

Assists in reviewing new associate applications, checks references, interviews applicants for the best possible hire.*

Assists in monitoring the associates' work and performance.*

Assists in coaching/counseling associates in a respectful manner.*

Assists in completing the associate performance evaluations in accordance with established policy.*

Assists in monitoring expenditures weekly to ensure compliance with department budgeted monthly amounts.*

Assists in scheduling associates based upon budgeted FTE's and prepares assignments to ensure efficiency.*

Provides "hands on" care on an as needed basis to meet the needs of the residents.*

Assists in performing evaluation for residents to determine level of care and services.*

Participates in monthly ECC & LNS meeting with Director of Resident Services and Corporate Director of Nursing.*

Ensures medication orders are accurate on the MORS.*

Assists in reviewing and investigating all incidents reports and recommends appropriate action/follow up to the Executive Director.*

Conducts cart audits ensuring accuracy and proper documentation.

Prepares 1823 forms for new residents, and updates / keeps current 1823 forms for existing residents.

Reviews resident charts and properly files old/outdated records and information.

Sets up new admission charts.

Prepares staff schedules and posts/makes available in a timely fashion..

Understands and adheres to the ALF regulations pertaining to assisted living health care laws.*

Maintains a safe and secure working environment free of objects/situations that could cause harm and/or falls to residents, families, visitors, and associates.*

Practices good body mechanics and safe working habits.*

Knows, understands, and follows established policy and procedures in emergency situations.*

Is considered essential personnel especially during emergencies.*

Wears clean and appropriate dress per established policy.*

Understands and complies with HIPAA regulations.*

Takes initiative in work, seeks out additional information and offers solutions/suggestions.

Willingly takes on special projects or assignments outside of regular duties.

Performs all other reasonable duties as assigned or as requested.

Supervisory Responsibilities:

Directly supervises employees in the Resident Services and Nursing Departments. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.

Qualifications:

Language Ability:

Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Math Ability:

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

Reasoning Ability:

Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.

Computer Skills:

To perform this job successfully, an individual should have knowledge of word processing software, electronic medical records.

Education/Experience:

High school diploma or general education degree (GED); and one to two years related experience and/or training; or equivalent combination of education and experience.

Certificates and Licenses:

Valid Florida Nursing License
