The Senior Clinical Director under the direction of the Chief MedicalOfficer and in collaboration with the Chief Operating Officer isresponsible for directing the operations, programs, and activities inthe clinical area, and is also responsible for planning, andcoordinating patient care activities to maintain standards of patientcare and advise medical staff in matters related to clinical services.
The position also leads the care delivery team in providingintegrated care utilizing a multidisciplinary approach to care forexisting patients, transition of care patients, high risk patients,and patients that are out of care. This requires working closely withthe Director of Integrated Services, Case Managers, Clinicians, andother members of the complex care team.
The role supports the provision of the day-to-day supervision ofclinical staff. Defines and maintains facility standards in nursingcare and practice in accordance with all regulatory requirements andaccreditation standards. The Senior Clinical Director is responsiblefor building and maintaining a clinic culture focused on the missionand vision of SHS including the Patient Centered Medical Home (PCMH)care delivery model, like the facilitation of daily team huddles andbi-weekly team meetings.
The Senior Clinical Director also acts in leadership capacity tocoordinate patient flow to assist with meeting the success of SHSstrategic objectives and mission as they relate to patient access andsatisfaction, providing quality care, and the optimal use of SHSprocesses, staff, and resources.
The Senior Clinical Director, working closely with the CMO and COO,is responsible for developing and facilitating the quality assurance,risk management, regulatory compliance, and operational performanceimprovement activities for all clinical services. Specifically, theefficient use of EMR, meaningful use and other clinical outcomemeasures, PCMH
requirements, and other clinical quality related activities. Theposition also supports the office of the CMO in leading audits andpeer review processes, in addition to providing support in thegathering and interpretation of relevant clinical data.
Maintains effective, collaborative working relationships with the VP,Compliance, Privacy and Information Security.
Experience and Skills: Essential Functions:
Function as a Clinical Provider a minimum of 2 sessions per weekand as needed to facilitate access to care.
Directs the day-to-day operations of clinical services division.
Supervises the Clinical Care Coordinator who provides daily carecoordination, consultation and intervention to patients with one ormore chronic diseases. Responsible for identifying said populationvia provider/external health facility referral, utilizationmanagement referral, disease registry reporting mechanisms andpatient self- referral.
Takes a leadership role in developing and implementing acontinuous quality assurance program that focuses on high qualityclinical care through the Patient Centered Medical Home (PCMH) caredelivery model, patient satisfaction, risk management, compliance,and patient safety.
Reviews Center performance and effects changes as necessary toimprove services, optimize resources, reduce risk, and assurecompliance with regulatory requirements.
Develops a positive relationship with area medical providers, andother medical or behavioral health service organizations to ensureaccess to care for Center patients and an effective referral andconsulting process.
Fosters a culture oriented to achieve the Center’s mission toserve the underserved in a primary care setting while maximizing theuse of limited resources.
Manages all vendor relations, training, and in-services
Manages all student externships and internships- with support fromHuman Resources
Immediately investigates patient complaints and incident
Ensures that policies and procedures of the Center are followedand that the Center is prepared for surveys/inspections.
Oversees and ensures clinical policies are appropriate andcurrent. Assist with the oversight of Risk and Compliance relatedpolicies and procedures as directed.
Develops staff to achieve high quality services with a high levelof patient satisfaction and efficiency.
Maintains and submits required data for reports, such as for theUnified Data Submission to HRSA and provider productivity report.
Selects, interviews and hires applicants for open positionsrelated to clinical services and coordinates job-specific trainingfor new hires.
Performs annual employee evaluations and competency assessments.
Attends required meetings and participates on committees as directed.

Board Certified Licensed MD, DO
Three years minimum managerial experience
FQHC experience preferred
