Job Description
LPN - Overview:
+ Work directly with supervising Registered Nurse
+ Provide patient care according to clients care plan and needs
+ Teach the client and/or family members about care and support skills
+ Communicate regularly with RN Case Managers, Clinical Care Coordinators, and Interdisciplinary Care Team
+ Accurately and thoroughly document patient care observations and interventions on the day services are rendered
+ Participate in Performance Improvement activities as assigned. Communicate information that enable collection and root cause analysis of data to identify opportunities for clinical improvement
+ Be an advocate for your patients and their families as you provide the tools and training to allow them to maintain a safe quality of life
LPN Qualifications:
+ Licensed as a License Practical Nurse in the state of Illinois in good standings
+ Valid Driver's License and reliable vehicle with current insurance
+ Demonstrated ability to work as a team with supervising RN and interdisciplinary team
+ Prior Nursing experience in a Home Health, Hospital, Rehab, or ALF setting is preferred
+ Experience with Electronic Health Records (currently using Kinnser software)
+ Comfortable with technology and must have a smart phone or mobile device
+ Current CPR certification
+ Strong communication and interpersonal skills
+ Great with customer service
